- Samenvatting
Verwekker: Humaan papillomavirus.
Besmettingsweg: Seksueel contact
Incubatietijd: 2-3 jaar tot een hooggradige CIN-laesie, 12-15 jaar naar een cervixcarcinoom.
Besmettelijke periode: Hangt samen met de periode waarin de infectie productief is (virusuitscheiding).
Maatregelen: Voorlichting
Symptomen: Een groot deel verloopt onopgemerkt. (CIN)-laesies met (contact)bloedingen, pijn en/of een branderig gevoel.
Preventie: vaccinatie in het RVP (en buiten het RVP op eigen kosten)
~~~~~~~~
De besmettelijke periode is niet goed vast te stellen. Bij aanwezigheid van laesies is tot 10 dagen na ontstaan nog HSV aantoonbaar. Ook zonder aanwezigheid van laesies kunnen patiënten echter nog besmettelijk zijn door asymptomatische uitscheiding.
Bij asymptomatische HSV-1 of -2-seropositieven is virusuitscheiding gevonden in 2-8% van alle dagen, dus 7-28 dagen per jaar. Recente gegevens laten zien dat zich kortdurende perioden voordoen van (sub)klinische en asymptomatische reactivaties met ‘viral shedding’, zelfs tot 20% van alle dagen (Coreey, 2007).
Asymptomatische verspreiding: Het huidige bewijs ondersteunt het idee dat asymptomatische individuen in staat zijn om het virus te verspreiden, zoals blijkt uit zowel Duits als Aziatische studies. Tijdens vroege infectie is er een hoog niveau van virusuitscheiding door de keel regio. - www.myeloma.org by Barend van Zadelhoff
- Voor behandeling van herpes simplex-virusinfecties van de huid en slijmvliezen bij patiënten bij wie de immunologische afweer is gestoord, komt intraveneuze toediening van aciclovir het eerst in aanmerking. Door deze therapie kan de duur van genezing, pijn en virusuitscheiding met enkele dagen worden verkort. - NTVG by Barend van Zadelhoff
- Viral shedding refers to the expulsion and release of virus progeny following successful reproduction during a host-cell infection. Once replication has been completed and the host cell is exhausted of all resources in making viral progeny, the viruses may begin to leave the cell by several methods.[1]
The term is used to refer to shedding from a single cell, shedding from one part of the body into another part of the body,[2] and shedding from bodies into the environment where the viruses may infect other bodies.[3]
A person with a viral disease is contagious if they are shedding viruses. The rate at which an infected person sheds viruses over time is therefore of considerable interest. Some viruses such as HSV-2 (which produces genital herpes) can cause asymptomatic shedding and therefore spread undetected from person to person, as no fever or other hints reveal the contagious nature of the host during this kind of shedding. [10]. Another crucial component to viral shedding is whether or not the age of the individual infected plays a role in how long the individual will shed the virus. The University of Milan conducted a study on the A/H1N1/2009 influenza virus to determine if the shedding of the novel pandemic occurs for a longer time in youth than in adults. Only children who had symptoms that appeared two days before hospital attendance, were under 15 years of age, and did not face any serious complications were included in the study. After physical exams and nasopharyngeal samples from all of the positive cases, the results determined that the length of viral shedding (in days) does not correspond to age. The virus shedding was not related to age because there was no difference between the children in the different age groups. Contagiousness in this situation could last up to 15 days, which means that when viral diseases infect much of a localized population, the proper quarantine precautions need to take place to prevent further spread of the virus through viral shedding.
