This study is about public health in India and more specifically about HIV/AIDS.
The TB pandemic is further complicated by the spread of HIV/AIDS, with 5.2% of all TB cases related to HIV infection. In the last five years, the government has ****up-scaled*** the DOTS (Directly Observed Treatment Short-course) intervention, which is estimated to have saved half a million lives.
National estimates are that about 5 million people were living with HIV/AIDS at the end of 2004 and prevalence rates are estimated to be between 0.4% and 1.3%. The rate is increasing throughout the country, although there is considerable regional variation. WHO estimates that of 770,000 Indians in need of antiretroviral therapy, only about 35,000 were receiving the medications at the end of 2004.
Does "up-scaled" mean that the government has extended the DOTS program to the richest classes of the population? Or does it mean that they just extended/intensified this program?

You're right. The general concept is that, in some way, the Indian Government has enlarged, expanded or improved the program. Xie xie for the "agree".

: Saving generated in DOTS comes from out-patient treatment as this is done by -in my country for example- patient's relative who takes part in the short course.
