It is a table listing employees covered under a company insurance policy.

In some instances, a person's name is givenunder "NOMBRE Y APPELLIDO", but mostly it is shown as "Alta".
Does this translate as "registered" or something similar?

I think that your understanding is accurate and, if so, this would seem the way it would be expressed in a corresponding English-language document. I can't imagine what else "alta" would mean in such a context.
