The National Prescribing Centre defines medicines reconciliation as:
o Collecting information on medication history (prior to admission) using
the most recent and accurate sources of information to create a full and
current list of medicines, e.g. GP repeat prescribing record
supplemented by information from the patient and/or carer, and
o Checking or verifying this list against the current inpatient
prescription/summary care record and administration record in the
hospital, ensuring any discrepancies are accounted for and actioned
appropriately, and
o Communicating through appropriate documentation, any changes,
omissions and discrepancies

"The steps in medication reconciliation are seemingly straightforward.7 For a newly hospitalized patient, the steps include obtaining and verifying the patient’s medication history, documenting the patient’s medication history, writing orders for the hospital medication regimen, and creating a medication administration record. At discharge, the steps include determining the postdischarge medication regimen, developing discharge instructions for the patient for home medications, educating the patient, and transmitting the medication list to the followup physician. For patients in ambulatory settings, the main steps include documenting a complete list of the current medications and then updating the list whenever medications are added or changed."

- ricognizione terapeutica: (I FASE) è la raccolta di dati riguardanti il paziente, i suoi stili di vita, le sue patologie e la terapia farmacologica in atto (nome, posologia, modalità di somministrazione, orari di somministrazione, periodo di inizio della somministrazione...)
-riconciliazione terapeutica: (II FASE) il medico prima di una nuova prescrizione prende atto della scheda di ricognizione e confrontando la terapia in corso con quella necessaria a quel nuovo momento clinico effettua una riconciliazione. Sospende eventuali farmaci, ne aggiunge altri, rimuove le sovrapposizioni.
Da quanto esposto, la ricognizione sarebbe assimilibabile a una sorta di "medical history" ma non sono sicuro che possa esistere un termine in inglese più specifico.
