‘Special Schemes (also known as Special Address Comments)’;
What is the purpose of a Special Scheme?
The primary purpose of a Special Scheme is to provide officers and staff with critical Police information that they;
(a) NEED to know prior to attending the address (i.e. they cannot attend without being told this information;
(b) NEED to action as a result of a 999 or 101 call being made from the address or following police attendance at the address.

It seems to me that the information in these schemes/comments would either be read by the officer (from a database) or told to them by dispatch, when responding to a call, or logged by the officer, post-attendance. In any case, I'd use "observações", which is the general term for additional/special comments pertaining to a certain piece of information (and also how one calls the field in which these comments are to be written/inserted/logged).
As an example, see this text about the standardization of police reports:
"Observações acerca da narrativa ou do preenchimento do documento deverão ser lançadas em campo apropriado. Pelo projeto (PLS 65/06), ainda que não seja possível concluir qual delito foi cometido, deverá ser indicado o tipo provável e registrada a ressalva no campo das observações." https://www12.senado.leg.br/noticias/materias/2010/08/10/bol...
