Medication therapy should be divided into three phases (1) baseline, (2) titration, and (3) maintenance.
Baseline - Before the first pill is administered, baseline data on height, weight, blood pressure and heart rate should be collected. Some also recommend obtaining a baseline complete blood count. A selected rating scale should be completed by parents and teachers to serve as a baseline measure of the severity of core symptoms of ADHD. A choice of medication is made during this phase.

The minimal effective daily dose and optimal timing of medication administration should be determined before switching to a long acting medication. Dosage adjustments are made based upon the findings of repeated rating scales, teacher's verbal reports, parent comments and adverse effects experienced by the child. Treatment duration should also be planned on an individual basis as well.

These factors are showcased in a case study of a state of the art prevention programme in a South African mining community.1 This programme was well funded and backed by an impressive array of local and international experts. On paper it seemed ideal. It was initiated by a group of township residents concerned about rocketing levels of HIV (baseline research showed that levels of HIV were 22% among mineworkers, 68% among sex workers, and 50% among young people). The programme was managed by a representative group of "stakeholders" living or working in the local community-grassroots groupings, mining industry doctors and human resources personnel, private GPs, traditional healers, trade unionists, representatives of local and national health departments, biomedical and social scientists, and foreign donor agencies.
