Subgroups of patients with the highest relative risk for sudden cardiac death (SCD) include survivors of out-of-hospital cardiac arrest, heart failure (HF) and patients with low ejection fraction.
However, these subgroups are a small proportion of the total population burden of SCD and therefore make precise identification of those patients that may benefit most from ICD difficult.
It is important to note that there is a trade-off in targeting progressively higher risk subgroups of patients for an ICD. To increase the confidence with which a fatal event can be predicted in the target group, an increasing number of patients at lesser risk are excluded from planned interventions. Therefore, as the target group becomes smaller, despite the high individual risk of fatal events, it will contain a progressively smaller number of the total fatal events experienced in the entire post-MI population. Although an intervention may save many lives in a targeted group, there would be only a small impact on overall survival in the entire population.
