Following your formulation assessment on 01.01.2015 to see one of our clinicians, please find enclosed the written outcome of your meeting, you have been accepted for a one off appointment with the Peterborough and Borders Locality Team. A member of the team will be contacting you by a letter in due course and a copy of the assessment has been sent to your GP.

In the meantime, if you feel that your mental health is deteriorating please contact your GP.
