Medical report:

Subject: breast cancer

The patient had an FNA which showed atypia. She then underwent an excisional biopsy on November 2012 and the final histology was that of a high-grade carcinosarcoma, 2 cm in size with central cystic hemorrhage, high grade and single focus. Margins negative but close and the closest margin was 2.8 mm, ER negative/PR positive at 60% and HER/neu negative by IHC.

The patient required wider margins so she underwent a wider excision of the tumor bed with a left axillary sential node biopsy on January 2013.

Review of systems: Please refer to the history of present illness and all the other review of systems is negative.

Oh my gosh, you're absolutely right!! Late night answering of questions and reading the question poorly has put my 7 years of med school to shame COMPLETELY. Inexusable error. Clearly it's a good thing I don't practice anymore :)

@Sarai The review of systems pertains only to the functional inquiry, and as such constitutes part of the history, not the physical examination. Thus nothing is "examined" and there are no "findings". A positive review of symptoms means that the patient responded in the affirmative to one or more questions pertaining to symptoms that he/she could be experiencing.

Generally, when examining a patient, if the other systems were examined and there were no other findings, we will say "the review of other systems was negative", indicating that there are no other notable findings.

This is an example of medical jargon, proving yet again that medicine is a subculture with its own idiomatic lexicon that transcends technical and anatomical terminology.

First of all, it is helpful to know the format of a standard medical report which is:
History of current complaint.
Past medical history
Family History
Allergies
Medications
Functional inquiry (also known as REVIEW of SYSTEMS)
Physical examination
Impression (also known as differential diagnosis)

The "review of systems" is a series of questions posed to the patient to check for symptoms that might not have been mentioned by the patient and which may or may not have relevance to the current complaint. These questions are organized by "organ system". Examples of organ systems are the respiratory system, the cardiovascular system, the musculoskeletal system, etc.

A functional inquiry usually takes the form:
Ear nose throat
Respiratory
Cardiovascular
Gastrointestinal
Genitourinary
Musculoskeletal
Neurological
Endocrinological
Integument
etc

Each of the above systems has a series of standard questions. Typical questions for the genitourinary system, for example, would be:
Do you have a weak urinary stream?
Do you have to get up a night to urinate?
Do you have pain on urination?
Do you ever notice blood in your urine?
Do you have problems with erections?
If female, when was the last menstrual period, etc ?

The "review of systems was negative" is medical shorthand for saying that other than the stated symptoms already mentioned in the history of the present illness, the patient did not have any of the symptoms posed during the functional inquiry.

"review of systems" is often used in the singular, if nearly 160k ghits are to be believed.
