"Optical media" is a generic allusion to the refractive portions of the eye (cornea, lens, humours). Some degree of opacification occurs in acute glaucoma.

agree doctor_suz : I don't quite understand though - the patient is blind now and has a remarkably low intraocular pressure. What happened in the first place, was there intraocular bleeding?

I guess the retina is already permanently damaged, and the improved transparency is just an examination finding. Corneal edema and anterior chamber inflammation could be present and would have subsided after treatment, threby improving transparency. Also,
