medical license
A medical license is needed in most countries in order for a person to be allowed to practice medicine. It is what allows/permits him/her to do so. I think this comes closest to your original German term as used for example at:

: medical licence is "Approbation" in German, which is not the same as "Praxisbewilligung". To open a practice in Switzerland, you need both. Permission to practice would work, though.

A "Praxisbewilligung" in Switzerland (since this involves a "Kantonsarzt"...) is literally the right to practice in one particular canton, while a doctor's medical licence ("Approbation") is valid all over Switzerland and the prerequisite for being granted the right to practice anywhere in the country. Incidentally, in the French-speaking part of Switzerland, it is called just that: "droit de pratique".

For a clear difference between "Approbation" und "Praxisbewilligung", it might even be better to make this the "right to open a practice", because that's what it is all about. Of course, you need a medical licence to practice medicine in Switzerland as well, but it doesn't automatically give you the right to open a practice. That right might be denied to you for political or economic reasons.
