hospital admission information
I'm translating the first part of a patient's hospital report, titled "Dati di Arrivo." It lists a number of items including Priorità (verde), Modalità Lesiva, Modalità d'Arrivo, INAIL (si/no) and then we have this "Steso Rapporto" where the answer is No. This is followed by the last item "Viene Per" (Malore Generalizzato).
What's this about? How do we say it in English?
Thanks again for your help.

Report drafted (written)
It sounds as if they are asking whether a report on the case was written.
Note from asker:
Ciao Marco, thank you for this suggestion. I still don't understand what rapporto they mean ... there's no indication in the medical record ... but your explanation does make sense. |

: Maybe it means that a report was provided by whoever referred the patient.
