Cemetery

A cemetery by definition is a burial ground not in a churchyard.

I would simply omit "open-air".

There are 'rural' and 'natural' cemeteries but these don't seem to fit the historical context.

Cemetery located outside of the city

I believe that "campal" in this sense means "campo", not open-air. Because of overpopulation and bad sanitary conditions, many cities were forced to stop burying their dead in churches and instead bury them in cemeteries outside of the city or in the country. I would suggest "cemeteries located outside of the city" or "country cemeteries".
