I didn't offer any references for "índice de frecuentación", but it's definitely not an error and definitely means "consultation rate":

If it "were "frecuentación" one would expect some explanation as to who or whose services one resorts to. Frecuentación de la consultas, de los servicios .....

: It has to do with "frecuentar" ... to visit frequently or habitually. The context posted by Peter tells you that.
