In my mind in can mean either - genomgången as in gå igenom suggests to me that they have had a fracture and been treated for it. But as Blangstedt says it could also just mean experienced and the word genomgången is just wrongly used meaning not gå igenom but varit med om.
But I have translated several texts about osteoporosis where a fracture is interesting as an indication of brittle bones (benskörhet) and if it has been treated or not is not the important thing but the fact that it happened in the first place.
I would not worry so much, but say "elderly patients with a previous hip fracture".
