Translation - English A Case of Simultaneous Superior Vena Cava Reconstruction for an Invasive Thymoma and Open Heart Surgery
We report our experience with an extremely rare case of simultaneous operations: mitral valve replacement for mitral valve stenosis and superior vena cava (SVC) reconstruction and extended thymectomy for invasive thymoma. A 49-year-old man was admitted to our hospital to undergo surgery for mitral valve stenosis. Preoperative computed tomography (CT) scan revealed a tumor in the anterior mediastinum, and upon careful inspection, we strongly suspected a thymoma with SVC invasion. Complete resection was deemed possible, despite SVC invasion, and was performed simultaneously with open heart surgery for mitral valve stenosis. We first performed extended thymectomy, and then, for the SVC invasion, we continued the surgery using cardiopulmonary bypass. We then inserted venous cannulae into the right and left brachiocephalic veins. We could completely resect the tumor with SVC. We performed mitral valve replacement and subsequently, reconstructed the SVC using a 16-mm ringed expanded polytetrafluoroethylene (ePFTE) graft. The patient was discharged after satisfactory postoperative course. Two months later, he underwent radiation therapy. The final diagnosis was mixed type invasive thymoma (WHO classification) at stage III according to the Masaoka staging system.
