Answers to your question The comma: 'lateral' is almost a synonym of 'subapical'; it's almost redundant because if one thinks about it, it couldn't be anywhere else but lateral. The rule is that two adjectives in a modifying string are separated with a comma when they are close in meaning. It's a hair-splitting rule and the comma could be omitted. It might be smoother to say 'in a lateral subapical position'.
The determiners: definite article for 'apical' because there's only one apex; indefinite article for 'subatypical' because there could be many positions below the apex.

Muriel, I like your option, but do you think the comma before lateral is necessary? why? Also, the article in the first part is "the" (the apical position) and the article in the second part is "a" (in a subapical position). Does it sound OK?

Of course, 'one in the apical' means 'one in the apical position' - 'position' comes at the end of the phrase.
