ST probably says something like 'kontrola'. I would not use check up in a formal context. How about 'subsequent/further/next examination'?

not the same
There is a reason why the source text mentions both, because they are not the same. Many people have regular check-ups to follow up on a condition they had before or to confirm that they are healthy. An examination is the actual physical examination done by the doctor during a check-up or during any other appointment that is made for a specific complaint.
Discussion
Examination.
Subjective:....
Otoscopical....
Tympanogram...
Tone audiogram....
then it states the overall opinion and says which additional tests should be done, and then says that these test results should be brought to the examination.
I think Peter has explained it perfectly, and I have used "examination" because I think it is to be read by other medical professionals.
And you go for a (medical) check-up.
In your context, Peter's answer is very appropriate.
