Here's the sentence: "In caso di primo trattamento, è opportuno raggiungere la posologia completa dopo qualche giorno di trattamento, incrementando gradualmente il dosaggio". I don't think they're talking about first-line treatment in this context, I think they just mean the first time the patient is receiving the treatment, but I am not so sure. What do you think? Thanks!

For added clarity, I'd rephrase and say sth along the lines:
"When the treatment is first introduced/initiated, it is indicated/wise to gradually increase the dose over a day or two until the full dosage regimen is achieved."
"When the treatment is first introduced/initiated, it is indicated/wise to gradually increase the dose over a day or two until the full dosage regimen is achieved."

...Dimenticavo, Elizabeth: assolutamente sì, la tua interpretazione dell'italiano è giusta, si tratta di pazienti **che non hanno mai ricevuto prima** (naive, appunto) quel trattamento, è la prima volta che lo ricevono. Nulla a che fare con "first-line/first-choice treatment", :)
