I believe "six-monthly" is the correct term here if this report is produced every six months.

Six-month Report for May 2009 to October 2009 on the Project # XC-07 entitled Improvement of Hygienic - Sanitary Conditions and Access to Safe Water.

Please note a six-month [better to use the noun, instead of the adverb] report would make the second date 2009 as well. A report is "entitled". Spelling of "Hygienic"
