1. My name is XXX. I am the social worker for Adam.
2. I write this position statement in response to the request of the family that the maternal grandmother, Anna Iksińska, be assessed as a connected persons foster carer for Adam under schedule 4 of the Care planning Regulations (2010).
3. I have produced a positive schedule 4 assessment of Mrs Iksińska which I understand is filed separately.
