I bardzo proszę nie kalkować wyrazu "test", ponieważ EN test = PL badanie, próba, a nawet (w szkole): klasówka, praca klasowa itd.
Chyba, że test polega na zaznaczeniu jeden z kilku możliwości, np.: Czy Cheops to nazwisko:
astronoma,
rolnika,
pisarza,
króla?

Related to your questions below, we can provide the following feedback.
We usually refer to TRF (Test Report Form) or the Master TRF only for blank harmonized form-sheet of the Test Report. Once a TRF is used by a CBTL as part of an actual project, and the test results and evaluations are filled into the form, we call it a Test Report (TR) or more precise a CB Test Report (CB TR).
