Medical
Medical (general)
I've put this in the medical category as it is taken from a hospitalisation report, however, the use seems to be quite general in nature. Found in a table of general information about the patient and the doctors attending. It appears on a row all on it's own (the information hasn't been redacted on this row, it is the only entry on that row) below "nom".

summarised discharge summary
Could it be résumé de sortie standardisé? Without more context, it's a stab in the dark...

And of course I meant to write standardised instead of summarised - anyone know how to change this?

Discussion
It could be this, but why would it appear in that column?
