مقدمه:
دمانس به صورت اختلال پیشرونده عملکردهای شناختی در حالت هوشیاری کامل، تعریف می شود (1). شایعترین نوع دمانس که 50-60% کل موارد دمانس مربوط به آن است، دمانس نوع آلزایمر (بیماری آلزایمر) است (2) و بعد از آن، دمانس عروقی و نوع مخلوط آلزایمر و عروقی، به ترتیب بیشترین شیوع را دارند (1).
اگر چه علت اصلی این بیماری نورودژنراتیو هنوز به درستی شناخته نشده است (3) اما تحقیقات برای فهم مبنای مولکولی این بیماری ادامه دارد البته بنا بر تحقیقات صورت گرفته، عوامل مختلف ژنتیک، بیونوروشیمیایی، نوروآناتومیک، ایمونولوژیک، متابولیک و سایکونورواندوکرینولوژیک در آسیب شناسی این بیماری نقش دارند (1).
از دیدگاه ایمونولوژیک، واکنشهای نابهنجار سلولی و سرمی در برابر نورونها و وجود آنتی بادیهای ضد بافت مغزی می تواند بر اثر یک عامل خارجی و یا اختلال خودایمنی درونزاد باشد و یکی از عوامل مطرح در این زمینه آنتی بادی پروتئین شوک حرارتی است (1).
گروه گسترده پروتئینهای شوک حرارتی، نخستین بار از بافتهای مختلف Drosophila با افزایش گذرای درجه حرارت مرکزی، تشخیص داده شدند (4).
البته به غیر از این مسئله، این پروتئینها در شرایط نامساعد نظیر هیپوکسی، ایسکمی و وجود عوامل استرسور مانند اندوتوکسینها، فلزات سنگین، حلال های آلی و اکسیدانها نیز تولید می شوند (5).
در خصوص اثرات این پروتئینها تا کنون نظرات مختلفی اعم از نقش پاتوژن و یا محافظ در برابر عوامل آسیب رسان به سلولهای بافتهای مختلف بدن بیان شده است (11-6).
Translation - English Comparison between the Levels of HSP27 Kda antibody in Patients with Alzheimer’s disease and Vascular Dementia
Introduction
Dementia is defined as progressive impairment of cognitive functions during full awareness (1). The most prevalent type is Alzheimer’s-type dementia which comprises 50-60 % of all cases (Alzheimer’s disease) (2). After Alzheimer’s-type dementia, vascular dementia and mixed dementia (Alzheimer’s-type and vascular dementia) are the most prevalent types, respectively (1).
Although the main cause of this neurodegenerative disease has not been properly indentified to date (3), research continues in order to figure out the molecular basis of this disease. It should be noted that according to research, different genetic, bioneurohemical, neuroanatomic, immunologic, metabolic, and psychoneuroendocrinological factors are involved in the pathology of this disease (1).
Immunologically speaking, abnormal cellular and serum reactions against neurons and the presence of anti-brain-tissue antibodies can be the result of an external factor or an endogenous autoimmune disorder. One of the factors considered in this area is HSP antibody (1).
The wide group of HSPs was first diagnosed from different drosophila tissues by increasing transient core temperature (4).
Moreover, these proteins are also produced in unfavorable conditions such as hypoxia, ischemia and the presence of stressor factors such as endotoxins, heavy metals, organic solvents and oxidants (5).
Regarding the effects of these proteins, different opinions have been expressed to date about the role of pathogens or protection against cell-damaging factors in various tissues of the body (6-11).

Source text - Persian (Farsi) تنباکوی غیر تدخینی تقریبا دارای 30نوع مختلف است که به صورت استنشاقی ،جویدنی، مکیدنی و یا قرارگیری در ناحیه لثه ها و دندانها مصرف می شود.2و1 این محصولات شامل ترکیبات سرطانزای متفاوتی از جمله نیتروز آمینهای خاص تنباکو ، کادمیوم، سرب، پولونیوم210، بنزوپیرین و فرمالدیید می باشند.3
در مطالعات متعددی ارتباط بین مصرف این محصولات و بروز سرطان دهان و حنجره در حیوانات آزمایشگاهی تایید گردیده است4و5. همچنین مصرف تنباکوی غیرتدخینی به عنوان ریسک فاکتور ایجاد سرطان دهان گزارش شده است 3و6و7.
با توجه به اینکه تشخیص زود هنگام کلید درمان سرطان دهان است کوشش در جهت بهره مندی از تستهای بیوشیمیایی حساس و قابل اعتماد در تشخیص زود هنگام بیماری بسیار قابل توجه است 8. محققین مختلف افزایش سطح سرم9و10و11 و بزاقی8و10و12 سیالیک اسیدها را در مبتلایان به سرطان دهان گزارش نموده اند. سیالیک اسیدها، ترکیبات قندی انتهایی زنجیره اولیگو ساکارید گلیکوپروتئینها و گلیکولیپیدها هستند که بعنوان ترکیبات مهم غشاء سلولها نقش مهمی در ایجاد رفتارهای نئوپلاستیک ایفا می نمایند13.
نقش این ماکرومولکولها درشناسایی و تاثیر سلولها بریکدیگر و ایجاد چسبندگی سلولی که در ظهور تغییرات بدخیمی اهمیت دارد، بسیار قابل توجه است. گلیکوزیلاسیون (Glycosylation)غیرعادی منجمله سیالیلاسیون (Sialylation)در غشاء سلولها حوادث مهمی در ایجاد تغییرات بدخیمی محسوب می شوند8. به عبارت دیگر افزایش مقدار سیالیک اسید روی سطح سلولهای تومورال و ترشح آن بوسیله بعضی از این سلولها غلظت آنها را در خون یا بزاق افزایش میدهد 13و14. مطالعات متعددی با نتایج ضد ونقیض در خصوص سطح سرمی و بزاقی سیالیک اسید در مصرف کنندگان محصولات تنباکو انجام شده است13و14و15و16.
با توجه به اثرات کارسینوژن تنباکوی غیر تدخینی در حفره دهان، مطالعات محدود و اندک در خصوص بررسی
سطح سیالیک اسید بزاقی در مصرف کنندگان تنباکوی غیرتدخینی بعنوان یک مارکر تشخیصی سرطان دهان، اهمیت تشخیص زود هنگام سرطان دهان، ترکیبات و محتوای متفاوت تنباکوی غیرتدخینی بر حسب منطقه مورد استفاده ،انجام گرفته است. لذا مطالعه مورد نظر با هدف بررسی سطح سیالیک اسید بزاقی در مصرف کنندگان تنباکوی غیر تدخینی در این منطقه صورت گرفته است.
Translation - English There are almost 30 different types of smokeless tobacco (ST) which are consumed via sniffing, chewing, sucking, or dipping (1, 2). These products include different carcinogenic compounds such as tobacco-specific nitrosamines, cadmium, lead, polonium 210, benzopyrene and formaldehyde (3).
The relationship between the consumption of these products and oral and laryngeal carcinoma in laboratory animals has been confirmed in several studies (4, 5). Moreover, the consumption of ST is reportedly a risk factor of oral cancer (3, 6, 7).
With regard to the fact that early diagnosis is the key to the treatment of oral cancer, remarkable efforts are being made to benefit from reliable sensitive biochemical tests for this purpose. Different researchers have reported increased serum (9, 10, 11) and salivary (8, 10, 12) levels of sialic acids in oral cancer patients. Sialic acids are the sugar compounds at the end of oligosaccharide glycoprotein and glycolipid chains which play an important role in the development of neoplastic behaviors as important compounds of cell membranes (13).
These macromolecules play a significant role in the identification and influence of cells on each other and creation of cell adhesion which is important in the emergence of malignant transformation. The role of unusual glycosylation including sialylation in cell membranes is considered important in the development of malignant transformation (8). In other words, increased amount of sialic acid on the surface of tumoral cells and its secretion by some of them increases its concentration in the blood or saliva (13, 14). Several studies with contradictory results are conducted on the serum and salivary levels of sialic acid in the consumers of tobacco products (13, 14, 15, 16).
With regard to the carcinogenic effects of ST on the oral cavity, few limited studies have been conducted on the serum level of sialic acid in ST consumers as a diagnostic marker of oral cancer, the importance of early diagnosis of oral cancer and different regional ingredients and contents of ST. Therefore, the present study was conducted to study the salivary levels of sialic acid in the consumers of ST.
