Unstable angina, non-ST segment elevation MI (NSTEMI): 300 mg loading dose; initiating therapy without loading dose will delay establishment of antiplatelet effect by several days; followint the loading dose, administer 75 mg/day for up to 12 months; may administer beyond 12 months if used in combination with aspirin (75-100 mg/day); long-term combination therapy with aspirin, following stent placement, is individualized depending on how a patient tolerates long-term dual antiplatelet therapy (DAPT), whther they have stable coronary artery disease, and do NOT have risk factors (eg, TIA of stroke, age >75 years, bleeding risk, low body wt, concurrent medications).

A pig, who heard these words, said, "Nothing like being short and fit; look, how short I am!"
The camel said, "Well, if I fail to prove the truth of what I said, I shall give up my hump."
The pig said, "If I fail to prove the truth of what I have said, I shall give up my snout."
"Agreed!" said the camel.
"Just so!" said the pig.
They came to a garden. It was enclosed by a low wall without any opening. The camel stood on this side the wall, and reaching the plants within by means of his long neck, made a breakfast on them. Then he turned jeeringly to the pig, who had been standing at the bottom of the wall, without even having a look at the good things in the garden, and said, "Now, would you be tall, or short?"
Next they came to another garden. It was enclosed by a high wall, with a wicket gate at one end. The pig entered by the gate, and, after having eaten his fill of the vegetables within, came out, laughing at the camel who had had to stay outside because he was too tall to enter the garden by the gate, and said, "Now, would you be tall, or short?"
Then they thought the matter over, and came to the conclusion that the camel should keep his hump and the pig his snout, observing, "Tall is good where tall would do; of short, again, it's also true."

—¡De acuerdo! — asintió el camello.
—¡Me parece justo! —contestó el cerdito.
Llegaron a un jardín. Estaba rodeado por una pared baja y sin puertas. El camello se paró al lado de la pared y, alcanzando las plantas gracias a su largo cuello, se las preparó de desayuno. Después, miró burlonamente al cerdito, que se había quedado al pie de la pared sin poder siquiera ver las delicias que había en el jardín, y le dijo:
—Y ahora, ¿qué preferirías? ¿Ser pequeño o grande?
Más tarde, llegaron a otro jardín. Estaba rodeado por una pared elevada que tenía una puertecilla de un lado. El cerdito entró por la puertecilla y, después de haberse comido una buena porción de los vegetales que había dentro, salió y, riéndose del camello que había tenido que quedarse afuera porque era demasiado grande para entrar al jardín por la puerta, le preguntó:
—Y ahora, ¿qué preferirías? ¿Ser grande o pequeño?
Después de pensar un poco en la pregunta, los dos llegaron a la conclusión de que el camello se quedaría su joroba y el cerdito su trompa, porque habían entendido que:
“Ser alto va bien donde sea que haga falta;
ser bajo, a su vez, también es ventaja.”
