How This Book Is Organized
This text is divided into two sections: one dedicated primarily to describing the structure and function of the cardiovascular system, and one devoted to detailing the effects of exercise on the system. The first section focuses on cardiovascular physiology and provides a concise description of the structure and function of each component of the cardiovascular
system—namely, the heart, the vasculature, and the blood. The first chapter is an integrative chapter on the normal function of the cardiovascular system that provides a theoretical foundation for the detailed discussions that follow and emphasizeshow the various
components of the cardiovascular system function together as an intact and highly interdependent organ system, both at rest and during exercise. Chapter 2 presents the heart as a pump and emphasizes the role of the heart in delivering oxygen-rich blood to the body, as well as the need to regulate cardiac output to match the metabolic demands of the body. Chapter 3 details the structure and function of the myocardial cells,
the myocytes, that are ultimately responsible for the contractile force of the heart. Chapter 4 addresses the electrical activity of the heart, both within the specialized conduction system of the heart and within the cardiac myocytes. Chapter 5 describes the standard electrocardiogram (ECG) and details the clinical relationship between electrical activity
in the heart and the waveforms visible on the ECG.
Chapter 6 provides an organ-level description of the function of the vasculature, discussing the important topics of hemodynamics in general and the regulation of blood flow and blood pressure in particular.
Chapter 7 delves into the relatively new science of vascular biology and details the structure and function of the endothelium and the vascular
smooth muscle. This chapter relies heavily on relatively recent discoveries to describe how substances released by the endothelium can control vessel
diameter and, ultimately, blood flow. Chapter 8 details the hemostatic function of blood, describing platelet function, coagulation, and fibrinolysis.
This chapter emphasizes the delicate balance that must be maintained between coagulation and fibrinolysis to prevent unnecessary blood clotting
while simultaneously being able to prevent blood loss when a vessel is damaged.
The second section of the book systematically details the effect of exercise on the cardiovascular system—including acute response and chronic adaptations to aerobic and resistance exercise. Chapter 9 describes the effect of acute aerobic exercise on cardiac function, vascular function, and hemostatic variables. Chapter 10 presents the chronic effects
of a systematic program of aerobic exercise training on cardiac structure and function, vascular structure and function, and hemostatic variables.
Following the same pattern, chapter 11 describes the effect of an acute bout of resistance exercise on cardiac function, vascular structure and function, and hemostatic variables. Finally, chapter 12 documents the chronic effects of a systematic program of resistance exercise training on cardiac structure and function, vascular structure and function, and
hemostatic variables.

Source text - English Multiple sclerosis (MS) is a chronic, multifactorial disease of the central nervous system (CNS), typified by repetitive relapses and/or progression. Oxidative stress is a crucial factor in MS pathogenesis by ameliorating leukocyte migration, contributing to oligodendrocyte damage and axonal injury. Reactive oxygen species (ROS) and reactive nitrogen species (RNS) are created in CNS of MS patients mainly by activated macrophages and microglia structures responsible for demyelinisation and axons disruption. As there is no cure for MS, caring for patients takes a multifaceted approach; addressing pathogenesis, symptoms associated with disease, and the emotional/lifestyle implications of the disease.
Translation - Russian Рассеянный склероз (РС) – хроническое прогрессирующее многофакторное заболевание центральной нервной системы (ЦНС), с характерными рецидивами. В патогенезе рассеянного склероза одним из факторов выступает окислительный стресс, ускоряя миграцию лейкоцитов, провоцируя повреждение аксонов и олигодендроцитов. У пациентов с рассеянным склерозом активные формы кислорода (АФК) и активные формы азота (RNS) воздействуют на ЦНС при участии (альтернативно) активированных макрофагов и клеток микроглии, усиливая демиелинизацию и разрушение аксонов. Поскольку рассеянный склероз не поддается излечению, уход за пациентами требует комплексного подхода; учитывается патогенез, симптомы, ассоциированные с заболеванием, образ жизни, эмоциональное состояние и последствия болезни.
English to Russian: Outcomes of pregnancy in women with cystic fibrosis (CF) taking CFTR modulators - an international survey General field: Medical Detailed field: Medical (general)
Source text - English Background: As their long-term prognosis improves, women with CF are increasingly choosing to have children, but the safety of CFTR modulators in pregnancy and breastfeeding is currently unknown.
Methods: A survey was sent to lead clinicians of adult CF centres in Europe, the United Kingdom (UK), United States of America (USA), Australia and Israel requesting anonymized data on pregnancy outcomes in women using CFTR modulators before and during pregnancy and lactation.
Results: We identified 64 pregnancies in 61 women taking IVA (n = 31), LUM/IVA (n = 26) or TEZ/IVA (n = 7), resulting in 60 live births. In 44 pregnancies, CFTR modulators were either continued throughout pregnancy or temporarily stopped and then restarted. Two maternal complications were deemed related to CFTR modulator therapy; cessation of modulator therapy resulted in clinical decline in 9 women prompting resumption of therapy during pregnancy. No modulator-related complications were reported in infants exposed in utero and/or during breastfeeding.
Conclusions: CFTR modulators were reported to be generally well tolerated in pregnancy and breastfeeding, with only 2 maternal complications that were deemed related to CFTR modulator therapy. Women stopping CFTR modulators in pregnancy may experience a decline in clinical status and in the cases identified in this survey, restarting therapy led to a clinical improvement. Current experience remains limited and longer-term prospective follow-up is required to exclude delayed adverse effects.
Translation - Russian Введение: при улучшении отдаленного прогноза, женщины с муковисцидозом хотели бы иметь детей, но в настоящее время ещё недостаточно изучена безопасность CFTR, – трансмембранных регуляторов проводимости при муковисцидозе: во время беременности и лактации.
Методы: чтобы собрать анонимные данные об исходах беременности у женщин, использующих модуляторы CFTR до, во время гестации и при лактации, был создан опрос, направленный ведущим практикующим клиницистам центров исследования и лечения муковисцидоза Соединенных Штатов Америки (США), Австралии и Израиля.
Итоги: Выявлено 64 случая наступления беременности у 61 женщин: принимавших Ивакафтор (31 случай), Люмакафтор+Ивакафтор (26 случаев), Тезакафтор+Ивакафтор (7 случаев), в итоге зафиксировано 60 живорождений.
В 44 случаях приём CFTR был либо прекращён, либо временно приостановлен до момента возобновления курса лечения муковисцидоза.
Два осложнения у матерей были только предположительно связаны с терапией CFTR; у 9 женщин прекращение приёма привело к значительному ухудшению проявлений муковисцидоза, что стало основанием для возобновления терапии на протяжении беременности.
Заключение: сообщается, что модуляторы CFTR в целом хорошо переносятся при гестации и лактации, а осложнения, связанные с терапией CFTR наблюдались только 2 у беременных. Прекращая прием модуляторов CFTR при беременности, женщины могут испытывать ухудшение состояния в отношении симптомов муковисцидоза, и в случаях, выявленных в этом исследовании, возобновление терапии привело к клиническому улучшению. Полученный опыт остается неполным, и для исключения побочных эффектов в отдаленной перспективе требуется проспективное исследование.

Source text - English The IOL is intended to be placed by a trained ophthalmic surgeon in the lens capsule in the posterior chamber of the eye, replacing the natural crystalline lens. This position allows the lens to function as a refractive medium in the correction of aphakia. This IOL has a biconvex optic containing an aspheric design and a diffractive structure on the anterior surface. The diffractive structure divides incoming light to provide a range of vision from distance to intermediate to near. This IOL provides an option for clinicians to provide patients an intermediate add power of +2.17 D and a near add power of +3.25 D.
Some adverse reactions which have been associated with the implantation of IOLs are: hypopyon, intraocular infection, acute corneal decompensation, and secondary surgical intervention. Secondary surgical interventions include, but are not limited to: lens repositioning, lens replacement, vitreous aspiration or iridectomy for pupillary block, wound leak repair, and retinal detachment repair.
To evaluate postoperative changes in retinal capillary plexus and to assess contributing factors in postoperative visual improvement using optical coherence tomography angiography (OCT-A) in patients with idiopathic epiretinal membrane (iERM) post membrane removal. Patients scheduled for vitrectomy and membrane peel for iERM were enrolled. 35 subjects were included for this study. OCT-A was used to measure the FAZ related parameters and the superficial and deep capillary plexus layers using 3 mm × 3 mm scans. Measurements were taken before surgery and at every post-surgical follow-up. The unaffected fellow eyes were used as controls.
Coexistence of thyroid-associated ophthalmopathy (TAO) and ocular myasthenia gravis (OMG) is very rare. Little is known about the orbital histopathology associated with this condition. The patient was documented with a history of OMG and was treated with blepharoplasty to correct ptosis 3 years prior to presentation. Physical examination revealed right upper eyelid retraction resulting from the eyelid surgery. Computed tomographic scan demonstrated bilateral enlargement of the extraocular muscles. Thyroid function test confirmed hyperthyroid status.

К побочным реакциям при имплантации ИОЛ, относятся: гипопион, внутриглазная инфекция, острая декомпенсация роговицы и повторная операция. Повторные хирургические вмешательства показаны при следующих состояниях (но не ограничиваются ими): изменение положения хрусталика, его замена, аспирация стекловидного тела или иридэктомия при зрачковом блоке, устранение истечений из надреза и коррекция отслойки сетчатки.
Оценка послеоперационных изменений в хориокапиллярном слое сосудистой оболочки и факторов, способствующих улучшению зрения в послеоперационный период, с помощью оптической когерентной томографии (ОКТ-ангиография) у пациентов с идиопатической эпиретинальной мембраной (иЭРМ) после её удаления. Оценивались пациенты с назначенной витрэктомией и отслойкой мембраны на фоне иЭРМ. В это исследование были включены 35 испытуемых. ОКТ-ангиография проводилась для измерения параметров, связанных с фовеальной аваскулярной зоной (ФАЗ), также с помощью сканирования размером 3 × 3 мм изучались поверхностные и глубокие хориокапиллярные слои сосудистой оболочки. Измерения проводились перед операцией и при каждом последующем наблюдении. Контрольное сравнение проводилось с группой пациентов без офтальмологического вмешательства.
Тиреоидная (эндокриннная) офтальмопатия, и миастения Грэвис встречаю довольно редко. Гистопатология в отношении этих заболеваний освещена недостаточно. У пациентки в анамнезе: миастения Грэвис, и блефаропластика для коррекции птоза за 3 года до наблюдения. Физикальное обследование выявило ретракцию века верхней правой части, как следствие блефаропластики. При компьютерной томографии выявлено двустороннее увеличение глазодвигательных мышц. Оценка функции щитовидной железы показала гипертиреоз.

Joints
With the exception of the hyoid bone in the neck, all bones form a joint with at least one other bone. Joints are also called articulations.Joints have two functions: (1) to hold the bones together; and (2) to give the rigid skeleton mobility. When two bones meet, or articulate, there may or may not be movement, depending on: (1) the amount of bonding material between the bones; (2) the nature of the material between the bones; (3) the shape of the bony surfaces; (4) the amount of tension in the ligaments or muscles involved at the joint; and (5) the position of the ligaments and muscles.
Part One—Classification of Joints
Joints are classified in two ways: by function and by structure.
Function
The functional classification of joints focuses on the amount of movement allowed by the joint.
Immovable Joints (Synarthrotic)
Immovable joints are found mostly in the axial skeleton, where joint stability and firmness is important for the protection of the internal organs.
Slightly Movable Joints (Amphiarthrotic)
Like immovable joints, and for the same reason, slightly movable joints are also found mainly in the axial skeleton.
Freely Movable Joints (Diarthrotic)
Freely movable joints predominate in the limbs, where a greater range of movement is required.

Source text - English Shoulder (Glenohumeral) JointType of JointSynovial ball-and-socket.ArticulationThe head of the humerus articulates with the shallow pear-shaped glenoid cavity (fossa) of the scapula. This articulation is inherently unstable because the glenoid cavity is only approximately one-third the size of the humeral head, although it is slightly deepened by a rim of fibrocartilage called the glenoid labrum or labrum glenoidale (triangular in cross section). The shoulder joint is the most freely moving joint of the body, precisely because stability has been sacrificed to enable maximum range of movement.
Articular Capsule
The articular capsule extends from the margin of the glenoid cavity (including part of the labrum) to the anatomical neck of the humerus. This thin capsule is very loose, thus enabling maximum movement of the joint. When the arm is by the side, the lower part of the capsule hangs in a loose fold, which becomes progressively tauter as the arm is abducted, and increasingly so if the arm continues into elevation. The capsule contributes very little to the stability of the joint. The surrounding muscles, whose attachments are intimately related to the capsule, largely provide joint stability.

Source text - English Functional Characteristics of a Skeletal Muscle
All that has been said about muscles so far in this book enables us to formulate a list of functional characteristics pertaining to skeletal muscle.
Excitability
Excitability is the ability to receive and to respond to a stimulus. In the case of a muscle, when a nerve impulse from the brain reaches the muscle, a chemical known as acetylcholine is released. This chemical produces a change in the electrical balance in the muscle fiber and, as a result, generates an electrical current known as an action potential. The action potential conducts the electrical current from one end of the muscle cell to the other and results in a contraction of the muscle cell, or muscle fiber (remember that one muscle cell = one muscle fiber).
Contractility
Contractility is the ability of a muscle to shorten forcibly when stimulated. The muscles themselves can only contract; they do not usually lengthen, except via some external means (i.e., manually), beyond their normal resting length (see “Tonus” below). In other words, muscles can only pull their ends together (contract); they cannot push them apart.

Here's my story of how, in 5 years, I went from being a freelance translator to a junior scientific editor and professional medical translator at the largest publishing house in Russia.
I cannot at the present moment recall when I began to translate. Although I have been doing it for about 6-7 years, it was not my full-time main job, but not anymore.
So let me tell you how it came about.
I was a student at a medical university in Russia, but my dreams of becoming a doctor did not come to true. I did, however, study anatomy, biochemistry, and more, as well as English, which had to be used to read medical literature.
It was at this time that the students were seeking the money they needed to get their education, so that my career as a translator of professional medical literature began. A few years later, in 2008, I graduated from the University of Medicine.

To this day, I am still working as a translator; however, In addition to my skills, I started working as a freelance interpreter. I want to note that I would love to avoid this. Truth be told I'm not too good against stress offline. I love books.
