following a request by: the concerned person
or shorter; as requested by: the concerned person. The difference between 'the interested' person and 'the concerned' person is that the request has to be made by the concerned person himself/herself while 'the interested person' can be in the name of the 'concerned' person.

I don't think you can use "interested" in this context - there are lots of parties who might be interested, such as potential employers. This has been requested by the person whose name is on the certificate.
