Abstract
Un primo obiettivo di ricerca è di tipo sperimentale e consiste nello sviluppare una procedura per correggere un difetto genetico del metabolismo epatico tramite il trapianto autologo (singenico) di precursori cellulari epatici, geneticamente modificati ex-vivo e successivamente reimpiantati.
La terapia cellulare, ovvero l’impiego di cellule a scopo curativo, è divenuta infatti oggetto di grande interesse nel corso dell’ultimo decennio, coinvolgendo una molteplicità di organi e tessuti grazie alle nuove conoscenze sulla biologia delle cellule staminali. Il nostro gruppo di ricerca ha già percorso la strada della terapia cellulare con impiego di epatociti, dalla sperimentazione animale fino all’applicazione clinica, e più di recente ha individuato una popolazione di cellule staminali di derivazione midollare con capacità di rigenerazione epatica in grado di correggere un difetto metabolico.
Al fine dello studio saranno utilizzate contemporaneamente metodiche di terapia cellulare e di terapia genica ex vivo. La prima consiste nel trapianto di singole cellule invece che del fegato intero mentre la seconda consiste nell’inserire ex vivo nelle cellule prelevate dal paziente i geni sani che sono in grado di correggere le alterazioni metaboliche. Questo modello è stato identificato nel ratto Gunn, che presenta iperbilirubinemia congenita da deficit dell’enzima UDP-glucuroniltransferasi , deputato alla coniugazione della bilirubina, analogamente alla malattia di Crigler-Najjar nell’uomo.
Si utilizzeranno popolazioni cellulari isolate da midollo osseo, in particolare: cellule con fenotipo
beta2m-/Thy-1 , in grado, una volta innestate in un modello animale, di differenziarsi in epatociti.
Le popolazioni cellulari isolate secondo le procedure precedentemente descritte saranno modificate geneticamente in vitro mediante trasduzione con vettori di origine virale in cui sia presente il gene terapeutico per l’UGT1A1 umano (compromesso nei pazienti affetti da Crigler-Najjar) e un marker, come il gene per la EGFP (Enhanced Green Fluorescence Protein), per rendere facilmente rilevabili le cellule trasdotte.
Le cellule saranno quindi reimpiantate nell’animale dopo aver indotto un danno ischemico epatico che favorisce l’integrazione delle cellule staminali trapiantate (I/R).Gli animali saranno poi sacrificati a distanza di 4 settimane e sarà valutata l’entità del ripopolamento epatico mediante analisi immunoistochimica della coespressione di antigeni epatici (albumina, CK8 e CK18) e della GFP.
Un secondo obiettivo dello studio riguarda la valutazione del ruolo dello stress ossidativo nel concorrere o nel determinare eventi di danno epatico in differenti condizioni morbose nell’uomo: epatopatie da virus, da alcool, da steatosi-epatica (NASH) da sindrome epatorenale, da fibrosi avanzata e nel fegato post-trapianto. I pazienti saranno valutati sia dal punto di vista istologico (grading; recettori per TxA2) mediante biopsia epatica, che dal punto di vista bioumorale (livelli plasmatici di F2-Iso; rilascio del ferro in forma libera; ossidazione lipidica; marcatori di stress infiammatorio), al fine di evidenziare se i parametri correlati allo stresso ossidativo risultano significativamente espressi rispetto alle altre cause di noxa epatica nel danno epatocellulare.

Parameters of oxidative stress in the pathogenesis of experimental ischemia/reperfusion injury and in metabolic hepatopathy and in post liver transplantation
Abstract
The first research aim is experimental and consists in developing a procedure to correct a genetic defect of hepatic metabolism through a syngenic transplant of genetically modified ex vivo hepatic cell precursors and subsequently reimplanted. Cell therapy, or more precisely, the use of cells for a curative purpose has become the subject of interest in the course of the last ten years involving a multiplicity of organs thanks to the new knowledge gained regarding the biology of staminal cells. Our research group has already travelled the road of cell therapy using hepatocytes, going all the way from the animal experimentation stage to clinical application, and more recently it has identified a population of staminal cells of bone marrow derivation with the capacity of hepatic regeneration capable of correcting a metabolic defect.
Methods of cell therapy and ex vivo gene therapy will be used contemporaneously at the end of the project. The first consists in the transplantation of single cells rather than the whole liver, while the second consists in inserting them ex vivo in the cells collected from a patient with healthy genes that are capable of correcting the metabolic alterations. This model was identified in the Gunn rat which presents congenital hyperbilirubinemia caused by the deficit of bilirubin UDP-glucuronosyltransferase responsible for the conjugation of bilirubin, as in the Criger-Najjar disease in man. Cell populations isolated from the bone marrow will be used, in particular, cells with beta 2m/Thy-1 phenotype capable of differentiation into hepatocytes. Cell populations isolated using the procedures described above will be modified genetically in vitro by means of transduction with vectors of viral origin in which the gene therapeutic for human UGT1A1, compromised in patients affected with Criger-Najjar syndrome, is present and a marker, such as the EGFP (Enhanced Green Fluorescence Protein), to make the transduced cells easily detectable.
The cells will then be reimplanted in the animal after hepatic ischemic injury has been induced to favour the integration of transplanted staminal cells ( ) The animals will be sacrificed 4 weeks later and the entity of hepatic repopulation will be assessed by means of immunohistochemical analysis of the coexpressions of hepatic antigens (albumin CK8 e CK18) and of the GFP.
The second aim of the study is to evaluate the role of oxidative stress in influencing or determining hepatic damage in different disease conditions in man: hepatopathies induced by virus, alcohol, non alcoholic steatoepatitis, the hepatorenal syndrome, advanced fibrosis, and in the post transplant liver. The patients will be evaluated both from the histological (grading, receptors for TxA2) by means of liver biopsy as well as biohumoral viewpoints (plasma F2 Iso levels free iron release, lipid oxidation, markers of inflammatory stress) in order to assess if the parameters correlated to oxidative stress are significantly expressed with respect to the other causes of hepatic noxa in hepatocellular damage.

I fell into this work quite by accident about 30 years ago when, in Italy on a hiatus after college, I found a job working for the director of one of the departments of the University Teaching hospital in Padua (Italy). One of my main duties was translating the papers that he and his collaborators wrote detailing their research procedures and results. Since then I have continued to do medical translations on an on and off basis and I have especially concentrated my energies there in recent years (last year - 2008 - approximately 20 of my translations/corrections were published).
Although quite different in every respect from scientific production, I can also boast experience in spiritual/humanistic literature as I worked for several years as an assistant editor of the English edition of the Messenger of St. Anthony.
It is vital that sentences in scientific texts are clear and appropriately linked, but the ebb and flow of words in a literary work enhances its meaning and impact.
