Source text - English Proton pump inhibitors increase gastric pH which can reduce the dissolution and absorption of oral posaconazole. Decreases in posaconazole Cmax and AUC by 46% and 32%, respectively, were observed in a study during coadministration with esomeprazole. A case report also describes decreased serum trough levels during use of posaconazole with omeprazole. If combination cannot be avoided, monitor the patient for reduced posaconazole efficacy during concurrent use and increase dose as necessary.
