• Your bleeding is so heavy that you are soaking a pad more often than every 2 hours after the first 24 hours after the birth, or more often than every 4 hours after that.
• You are unable to urinate or have any pain or burning with urination
• Your temperature is higher than 100.4
• Your uterine cramping is increasing instead of decreasing, and you have cramping even when you haven’t nursed.
• You have signs of breast infection-an area on the breast that is tender, red or hot.

Source text - English I, ___________________________, hereby consent to admission and care by Sioux Center Health Hospice. I acknowledge and consent to the following:
1. I have been fully informed of my terminal medical condition.
2. The hospice program and alternative methods of care and treatment have been explained to me.
3. While I am a patient in the hospice program, I will receive palliative care, which is care aimed at improving the quality of my life by relieving symptoms and discomfort, but not curative care directed at my terminal illness.
4. My hospice care will be under the medical supervision of the attending physician of my choice. The hospice medical director or his alternate will also be involved in supervision of my care.
5. I understand that as long as I am enrolled in this hospice program, an Interdisciplinary Group of caregivers will manage my medical care and social needs whether I am being cared for in my home or in a health care facility.
6. I give consent and approval for the release of information and medical records to or from any hospital, nursing home facility, private physician or insurance carrier.
7. I authorize licensed hospice personnel to be responsible for the disposal of unused Schedule II pharmaceuticals when no longer needed.
