Четыре года назад я поняла, что не справляюсь больше с жизнью. Мне невыносимо, невыносимо, невыносимо. Кроме этого, у меня был еще и спектр более понятных симптомов: от обонятельных галлюцинаций и дереализации до затяжных эпизодов депрессии и самоповреждения. Но непереносимость существования всегда беспокоила меня больше всего.
За эти четыре года специалисты в трех разных странах - Литве, Украине и Японии - конкретный диагноз мне так и не поставили. Звучали такие варианты как депрессия, биполярное расстройство, посттравматическое стрессовое расстройство и пограничное расстройство личности.
Мое близкое знакомство с психиатрией началось примерно за полгода до моей первой госпитализации. В то время я проходила практику по своей специальности психолога в глухой литовской психбольнице: проводила музыкальную терапию и участвовала в арт-терапии. Один из пациентов, увидев тогда мои рисунки, сказал: "Девочка моя, так тебе место в нашем заведении". А через шесть месяцев там я и оказалась. Правда, уже в Вильнюсе.
"Меня на работе не будет, я в больнице"
Получив направление в больницу, прежде всего позвонила своему менеджеру, чтобы объяснить прогул: "Привет, Нериюс. Меня на работе не будет, я ложусь в психиатрическую больницу. Да, и завтра тоже не будет. Депрессия, очень сильная". Звонить было очень страшно, чувствовала себя школьницей-прогульщицей, которая придумывает абсурдные оправдания. Голос моего собеседника звенел на другом конце трубки, отдавая тревогой. А смогу ли я выйти через неделю? А сколько будет длиться лечение? А собираюсь ли я оставаться в компании? После этого поехала в супермаркет купить булочки и яблоки. Затем собрала пижаму, какие-то предметы личной гигиены и направилась в больницу, расположенную в пригороде Вильнюса.
Очень четко помню, что в тот день было какое-то редкое затмение и очень страшный кровавый закат. В приемной больницы мне рассказали, что в дни наводнений и затмений у них всегда неспокойно: у многих пациентов существенно ухудшается состояние.
Через несколько дней, когда мне уже можно было пользоваться телефоном, мне позвонил какой-то большой начальник с работы. Мне сообщили, что, учитывая обстоятельства, я получу месяц оплачиваемого больничного, и меня очень ждут в офисе. Я ждала чего угодно, но не этого. Кому нужен сотрудник, который не справляется со своими обязанностями? К сожалению, долго проработать мне там все же не удалось: вскоре после первой госпитализации случилась вторая, и я решила вернуться в Украину.
После первого "взрослого" ряда эпизодов я не сразу поняла, что это у меня, кажется, серьезно. Мне казалось, что обе госпитализации были совершенно случайны и необоснованы. Более того - я свято верила, что намеренно обманула врачей, заставив поверить в свои вымышленные симптомы. А зачем обманывала? Внимания хотела, конечно.
Казалось, что с выпиской из больницы прекратятся и все эти отвратительные состояния. Очень хотелось в это верить. По крайней мере, увидев, как госпитализация разбила и истощила мою мать, я решила больше не "болеть". Мой расстройство стало для меня синонимом позора, инфантильности и ментальной лени.
Шутки о "дурке" и "чокнутых"
Вернувшись из Литвы в Украину, я немедленно начала искать работу. Отечная и полненькая от антипсихотиков, я начала ходить на собеседования - и вскоре устроилась малопонятным аккаунт-менеджером чего-то такого же малопонятного.
Из офиса убежала через два месяца: постоянная сенсорная гиперстимуляция, невозможность находиться среди людей, частые эпизоды деперсонализации. Моя карьерная модель стала приобретать четкие очертания: я приходила в новые места полной сил, но в течение первых нескольких недель непременно "возвращалась" в знакомое патологическое состояние. Нет, не в нормальный стресс первых дней работы. В непереносимость.
Вопрос своего психического здоровья в Украине с начальством и коллегами обсуждать не пыталась. Еще не видела атмосферы, в которой можно было бы безопасно об этом говорить, без необходимости каждый раз проводить лекции: да, психические расстройства существуют, нет, они не от безделья.
Шутки о "дурке" и "чокнутых" очень режут слух в украинских коллективах.
В Украине у меня практически нет вещественных доказательств моей болезни (болезней?), кроме пары ненужных рецептов на препараты. Психиатры, с которыми я виделась здесь, в унисон говорили: не нужны тебе никакие справки, только жизнь испортят. Сначала такое отношение казалось мне циничным или даже непрофессиональным, но сейчас я лучше понимаю прагматизм ситуации. В стране, где о психических расстройствах можно только шутить (и ни в коем случае не говорить серьезно), а поход к психиатру ставит на тебе пожизненное клеймо, пожалуй, действительно лучше обходиться без справки, пока на это есть силы.
Равнина синих деревьев
Приятно думать, что душевные страдания вызывают объективные, четко измеряемые внешние причины. Мне, например, всегда казалось, что я страдаю из-за того, что не могу найти свое место в жизни. А где чаще всего человеку приходит в голову искать свое место в жизни? Правильно - не здесь. "Не здесь" может сводиться к другой работе, другому городу или даже стране. Мое "не здесь" я нашла в Японии, в одной из многочисленных школ по изучению английского.
Япония, как и раньше, имеет один из самых высоких уровней самоубийств среди развитых стран, сообщает Всемирная организация здравоохранения. Здесь играют роль культура стыда, все большая социальная изоляция, жесткая стигма вокруг вопросов психического здоровья. А для иностранца еще и языковой барьер, отсутствие надежной системы социальной поддержки, неизбежный культурный шок, ограниченные возможности получения медицинской помощи.
Крошечные квартиры идеально подходят для того, чтобы переживать личную трагедию в одиночестве. А если не можешь пережить - то к твоим услугам всегда печально известный лес самоубийц у подножия Фудзиямы, Аокигахара. Равнина синих деревьев.
Устраиваясь на работу в японскую школу, я не решилась вспоминать о своем психическом состоянии - боялась потерять магический шанс поехать в Японию. Поэтому для руководства школы и сотрудников мои частые больничные стали неприятным сюрпризом. Опять болеет? Но у нее уже была простуда на прошлой неделе. И раменом она травилась уже. И мигрени у нее уже были. Странно.

В результате мой визит в частную "клинику ментального здоровья" в центре Роппонги длился всего пятнадцать минут: девять, чтобы заполнить бумаги, и шесть, чтобы поговорить с врачом. Где я вижу себя через пять лет?
Живя с биполярным расстройством (или предельным, или с депрессией, или что там у меня), очень трудно научиться себе доверять. Мне действительно плохо или я просто придумываю? Действительно ли это галлюцинации? Может, если бы я смогла немного сильнее постараться, быть немного настойчивее, чуть стабильнее, то вымышленные симптомы ушли бы, испугавшись моей настойчивости и желания справиться? К сожалению, симптомы не уходят.
Я до сих пор не овладела мастерством жить с психическим расстройством. Не знаю, как говорить об этом с людьми вокруг себя, да и стоит ли. Как совмещать работу и болезнь, где выкроить место для безболезненного саморазвития, как строить отношения.
Трудно примириться с самой собой. У меня внутри много ярости, разочарования, преданного честолюбия. Из-за этого трудно строить долгосрочные планы, отношения, давать обещания, мыслить. Где вы видите себя через пять лет, любят спрашивать на собеседованиях. Не знаю, не вижу.

In Ukraine, 1.2 million people suffer from mental health disorders—a number that continues to grow every year, according to the Ministry of Social Policy. On World Mental Health Day, observed on October 10, a Ukrainian woman named Sofia (name changed) shared her story of living with a mental illness.
Four years ago, I realized I could no longer cope with life. It was unbearable—truly unbearable. Along with that came a range of more recognizable symptoms: from olfactory hallucinations and derealization to prolonged bouts of depression, and self-harm. But what haunted me most was how unbearable it felt just to exist.
Over those four years, I’ve seen specialists in three different countries—Lithuania, Ukraine, and Japan. None of them have given me a definitive diagnosis. The possibilities have ranged from depression and bipolar disorder to post-traumatic stress disorder and borderline personality disorder.
My journey into psychiatry began about six months before my first hospitalization. At the time, I was interning as a psychology student at a remote psychiatric hospital in Lithuania. I led music therapy sessions and took part in art therapy. One patient saw my drawings and said, “Sweet girl, you belong here with us.” Six months later, I ended up in a place just like that, but in Vilnius.
“I won’t be coming to work—I’m being admitted to a hospital”
When I received my hospital referral, the first thing I did was call my manager to explain my absence: “Hi, Nerijus. I won’t be coming to work—I’m being admitted to a psychiatric hospital. I won’t be in tomorrow either. It’s depression. A severe one.” Making that call felt terrifying, like a schoolgirl making up a ridiculous excuse to skip class. His voice was tense with concern. Would I be back in a week? How long would the treatment take? Was I planning to stay with the company?
Afterward, I went to the supermarket for some bread rolls and apples. I then packed my pajamas and some toiletries and headed to the hospital on the outskirts of Vilnius.
I remember that day vividly. There was a rare eclipse and a frightening, blood-red sunset. Admissions staff told me that eclipses and floods always bring unrest: many patients experience a significant worsening of their condition. A few days later, when I was allowed to use my phone, a senior manager called. He said that, under the circumstances, I would be granted a month of paid sick leave and that they were really looking forward to having me back in the office. I was prepared for anything but that. Who wants an employee who can’t handle the job?
Sadly, I didn’t last long. Soon after my first hospitalization came a second, and I decided to return to Ukraine.
After the first few full-blown episodes, it didn’t immediately dawn on me that this might be serious. I felt like both hospitalizations were random and unjustified. What’s more, I was genuinely convinced that I had tricked the doctors into believing my imagined symptoms. And why? For attention, obviously.
I imagined that being discharged would mean the end of those horrible episodes too. I desperately wanted that to be true. Especially after seeing how broken and devastated my mother was by my hospitalization—I promised myself I wouldn’t “be sick” anymore. My disorder became synonymous with shame, immaturity, and mental laziness for me.
Nuthouse and Nutcase Jokes
When I returned from Lithuania to Ukraine, I immediately started looking for work. Still puffy and bloated from antipsychotics, I went to interviews and soon landed a vague position as an account manager for something equally vague.
After just two months, I fled the office. The constant sensory overload, the inability to be around people, the frequent episodes of depersonalization had become too much. A pattern quickly emerged: I would start a new job full of energy, only to inevitably spiral into the familiar pathological state within the first few weeks. Not normal new-job stress, but the unbearable kind.
I never attempted to discuss my mental health with my Ukrainian bosses or colleagues. I’ve never found a workplace where it felt safe to talk about it without having to give a defensive lecture every time: yes, mental illnesses are real; no, they’re not caused by laziness. “Nuthouse” and “nutcase” jokes hit especially hard in Ukrainian offices.
In Ukraine, I have almost no official proof of my illness (illnesses?)—just a couple of useless prescriptions. Every psychiatrist I’ve seen here has said the same thing: “You don’t need it certified. It’ll only ruin your life.” At first, I thought this was cynical or unprofessional. Now I understand the pragmatism. In a country where mental illness is a punchline (and never something people discuss seriously) and where seeing a psychiatrist can brand you for life, it’s often wiser to go without official papers—at least while you still have the strength to manage without them.
The Sea of Trees
It’s comforting to believe that emotional suffering comes from objective, clearly measurable external causes. I always thought my pain stemmed from not being able to find my place in life. And where do people usually look for that place? Anywhere but here. That “but here” might mean a different job, a different city, or even a different country. I found my “but here” in Japan, teaching at one of the many English language schools.
Japan, according to the World Health Organization, continues to have one of the highest suicide rates among developed countries. Shame culture, growing social isolation, and harsh stigma around mental health all play a part. For a foreigner, there’s also the language barrier, the lack of a reliable support system, culture shock, and limited access to healthcare.
Tiny apartments make the perfect setting for enduring a personal tragedy all by yourself. And if you can’t endure it—there’s always the infamous suicide forest at the foot of Mount Fuji: Aokigahara. The Sea of Trees.
When I applied for the job at the Japanese language school, I didn’t dare mention my mental health. I was afraid of losing the magical chance to go to Japan. So when I started taking frequent sick days, it came as an unpleasant surprise to my bosses and coworkers. Sick again? But she had a cold last week. And ramen poisoning. And migraines. How strange.
After yet another downturn, I finally decided to see a psychiatrist. It’s hard to describe the odyssey it took just to find an English-speaking psychiatrist in Tokyo. There are very few psychiatrists in Japan—and even fewer who speak any English.
In the end, my visit to a private mental health clinic in central Roppongi lasted only fifteen minutes: nine to fill out paperwork, six to speak with the doctor. The doctor (my fifth psychiatrist) couldn’t understand why no one was consistently monitoring or counseling me. He reluctantly wrote me a sick note, prescribed mood stabilizers, and asked me to return in two weeks.
I had to bring the note to work if I wanted to be paid for my missed days. The school director opened the envelope and said nothing. She didn’t speak English. I only saw her raise her eyebrows and curl her lip in something that looked like disgust. She had someone translate so she could speak to me. She asked the usual questions: Was I safe around children? Was I quitting? Was I taking medication? She promised not to tell the other teachers, but she shared everything with her most trusted subordinate that same evening.
A month of unbearable work followed—sideways glances from colleagues, increasing workload, and steadily worsening health. Then I was summoned to the central office for a talk. A month later, I attempted suicide—not in Aokigahara, but at home. Then I returned to Ukraine.
Where Do I See Myself in Five Years?
Living with bipolar disorder (or borderline personality disorder, or depression—whatever it is I have), it’s really hard to learn to trust yourself. Am I really unwell, or am I just imagining it? Are these really hallucinations? Maybe if I just tried harder—if I were more persistent, more stable—my “imaginary” symptoms would disappear, scared away by my persistence and determination to overcome them. Unfortunately, they don’t.
I still haven’t mastered the art of living with mental illness. I don’t know how to talk about it with the people around me, or even whether I should. I don’t know how to balance work and illness, where to find space for painless personal growth, or how to build relationships. It’s hard to make peace with myself. There’s so much rage, disappointment, and wounded ambition inside me. That makes it difficult to plan for the future, to build long-term relationships, to commit, to think.
“Where do you see yourself in five years?”—interviewers love to ask that question. I don’t know. I can’t see.

Я, участковый инспектор Усть-Ишимского РОВД УВД Омской области капитан милиции Мителев, рассмотрев заявления гражданки Погребовской, которая проживает в поселке Малая Бича по ул. Колхозная,
УСТАНОВИЛ:
Что Погребовская имеет в своем хозяйстве три поросенка в возрасте четырех месяцев и одну свиноматку в возрасте трех лет и одного кабана в возрасте шести месяцев. 17-го сентября 1979 года поросенок в возрасте 3-х месяцев по кличке «Боря» не вернулся вместе с другими поросятами. Мною было установлено, что Погребовская проживает на краю поселка «Малая Бича», и поросята паслись на берегу реки «Ягодка», которая впадает в Иртыш. Отбившись от основного стада, поросенок Боря пасся один вдоль берега р. Ягодка. На противоположном берегу колхозниками был посажен овес, который к тому времени уже вырос и выглядел аппетитным. Отказавшись кормиться травой, Боря решил переплыть через реку и попробовать овес, но долго не решался, так как на противоположном берегу ходили колхозники и могли его побить. В 12 часов колхозники ушли на обед и Боря решил плыть, но не учел направление ветра и силу течения, и Борю вынесло в реку Иртыш, но неразумное животное продолжало плыть против течения, надеясь, что выплывет, но не рассчитало свою силу и возможности. Стадо поросят видело, что он погибает, но оказать действенную помощь не смогло, при этом громко визжа, пытаясь тем самым привлечь внимание людей. Поросенок Боря длительное время барахтался в воде, при этом оглашая громким визгом окрестности поселка Малая Бича, и после того, как изнемог, покорился и тихо утонул.
Данную смерть поросенка Бори видели колхозники, которые убирали урожай, о чем подтверждают в своих показаниях. Таким образом кражи не было.
Руководствуясь статьей 113 УПК и статьей 5 пункта 1 УПК

В возбуждении уголовного дела по факту кражи поросенка Бори у гр. Погребковской отказать, о чем ее уведомить.

I, Militia Captain Mitelev, a precinct inspector of the Ust-Ishim District Department of Internal Affairs in Omsk Region, after reviewing complaints from citizen Pogrebovskaya, a resident of the village of Malaya Bicha, Kolkhoznaya Street,
FOUND THE FOLLOWING:
Citizen Pogrebovskaya keeps three four-month-old piglets, one three-year-old sow, and one six-month-old boar on her smallholding. On September 17, 1979, one of the piglets, a three-month-old named “Borya,” failed to return with the others. My investigation revealed that Pogrebovskaya lives on the outskirts of Malaya Bicha village and her piglets customarily grazed along the bank of the Yagodka River, a small tributary that flows into the Irtysh. The piglet Borya strayed from the rest of the herd and was grazing alone by the riverbank. On the opposite shore, collective farm workers had planted oats, which by then had grown tall and appeared appetizing. Refusing to graze on grass, Borya resolved to cross the river to sample the oats. He hesitated for some time, as collective farmers were still on the opposite bank and could potentially harm him. But at noon, when the workers departed for their midday meal, Borya attempted the crossing. However, he did not account for the direction of the wind and the strength of the current, which carried him into the Irtysh river itself. The irrational creature persisted in swimming upstream, hoping to reach the far bank, but misjudged his strength and endurance. The herd of piglets observed Borya’s struggle but were powerless to offer meaningful assistance. They could only squeal loudly in an attempt to draw human attention. The piglet Borya flailed in the water for a considerable amount of time, his distress echoing through Malaya Bicha village. But after his strength waned, he succumbed and drowned quietly.
This death of the piglet was witnessed by collective farmers engaged in harvest, and their testimonies confirm this account. Thus, no theft occurred.
NOW, THEREFORE, pursuant to Articles 113 and Article 5, Clause 1 of the Code of Criminal Procedure,
I hereby ORDER as follows:
The initiation of a criminal case regarding the alleged theft of the piglet Borya from citizen Pogrebovskaya shall be denied and the complainant shall be notified accordingly.

Source text - English Ivermectin is an FDA-approved broad spectrum anti-parasitic agent that in recent years we, along with other groups, have shown to have anti-viral activity against a broad range of viruses in vitro. Originally identified as an inhibitor of interaction between the human immunodeficiency virus-1 (HIV-1) integrase protein (IN) and the importin (IMP) α/β1 heterodimer responsible for IN nuclear import, Ivermectin has since been confirmed to inhibit IN nuclear import and HIV-1 replication. Other actions of ivermectin have been reported, but ivermectin has been shown to inhibit nuclear import of host and viral proteins, including simian virus SV40 large tumour antigen (T-ag) and dengue virus (DENV) non-structural protein 5. Importantly, it has been demonstrated to limit infection by RNA viruses such as DENV 1-4, West Nile Virus, Venezuelan equine encephalitis virus (VEEV) and influenza, with this broad spectrum activity believed to be due to the reliance by many different RNA viruses on IMPα/β1 during infection. Ivermectin has similarly been shown to be effective against the DNA virus pseudorabies virus (PRV) both in vitro and in vivo, with ivermectin treatment shown to increase survival in PRV-infected mice. Efficacy was not observed for ivermectin against Zika virus (ZIKV) in mice, but the authors acknowledged that study limitations justified re-evaluation of ivermectin's anti-ZIKV activity. Finally, ivermectin was the focus of a phase III clinical trial in Thailand in 2014–2017, against DENV infection, in which a single daily oral dose was observed to be safe and resulted in a significant reduction in serum levels of viral NS1 protein, but no change in viremia or clinical benefit was observed.

Останніми роками наша, а також інші дослідницькі групи, продемонстрували його противірусну активність in vitro щодо широкого спектра вірусів. Спочатку івермектин був ідентифікований як інгібітор взаємодії білка інтегрази (IN) вірусу імунодефіциту людини 1-го типу (ВІЛ-1) з гетеродимером імпортинів α/β1, який забезпечує транспортування IN до ядра клітини. Such model, however, is likely to rely on temporary state institutions rather than local self-governance bodies. Decision-making will probably follow a single-command structure rather than a collegial approach. It is expected to operate under heightened security risks and amid extensive infrastructure damage, with the majority of local residents displaced. Given these constraints, a full restoration of all prewar instruments of local democracy may not be feasible. Nonetheless, some of them could be adapted to support the efforts.

Source text - Russian Пациентка 38 лет обратилась с жалобами на колющие, ноющие боли в нижних отделах живота, пояснице, паховой области, внутренней поверхности бедер.
Боли носят рецидивирующий характер, количество обострений до 3–5 в год, преимущественно в холодное время года.
Из анамнеза известно, что в 2004 г. выполнена высокая субтотальная гистерэктомия без придатков в раннем послеродовом периоде по поводу гипотонического кровотечения, в 2006 г. – диатермоконизация шейки матки по поводу цервикальной интраэпителиальной неоплазии III.
По настоящее время сохраняются циклические менструальноподобные выделения.
За последние 3 года – более 12 госпитализаций в гинекологические и хирургические отделения стационаров города по поводу выраженного БС.
Диагнозы стационара:
«кишечная колика», «мезаденит», «гематоцервикс, болевая форма», «гематометра», «пельвиоперитонит», «хронический аппендицит».
Во время стационарного лечения получала анальгетики, спазмолитики, антибактериальную, инфузионную терапию, несколько раз выполнялось бужирование цервикального канала, дважды – диагностическая лапароскопия.
Ранее консультирована гинекологом, диагноз «состояние после субтотальной гистерэктомии, диатермоэксцизии, СХТБ».
Рекомендована симптоматическая терапия при болях.
Консультирована неврологом, диагноз «вертеброгенная люмбалгия».
Рекомендованы снижение массы тела, плавание в бассейне, анальгетики при болях, препарат Лирика в постоянном режиме.
При выполнении второй диагностической лапароскопии хирургом обнаружены инъекция сосудов и необычные изменения париетальной брюшины малого таза, выполнена биопсия в связи с подозрением на туберкулез, результат гистологического исследования – воспалительные изменения брюшины.
При морфологическом исследовании выявлены воспалительные изменения: нейтрофильная инфильтрация, повышение количества межэпителиальных лимфоцитов, лимфо-плазмоцитарная инфильтрация.
Пациентка направлена на дальнейшее консультирование к фтизиатру.
При дальнейшем обследовании у фтизиатра данных, подтверждающих внелегочную форму туберкулеза, не получено.
При физикальном осмотре у пациентки отмечаются типичные герпетические высыпания на ягодичной области.
Методом ПЦР в мазке-соскобе подтвержден ВПГ 2-го типа.
Выставлен диагноз «рецидивирующий генитальный герпес, тяжелое течение».
Назначена этиотропная противогерпетическая терапия в супрессивном режиме – валацикловир (Валвир) 500 мг 1 раз в день в течение года.
На фоне проводимой терапии в течение 12 месяцев пациентка жалоб не предъявляет.
Translation - English A 38-year-old female patient presented with sharp, aching pains in the lower abdomen, lumbar and inguinal regions, and along the inner thighs.
The pains were recurrent, with three to five exacerbations annually, mainly during the cold season.
Her medical history reveals a high subtotal adnexa-sparing hysterectomy, performed in the early postpartum period in 2004 due to hypotonic bleeding, and an electrosurgical conization of the cervix in 2006 for grade 3 cervical intraepithelial neoplasia.
She continues to experience cyclical, menstruation-like discharge.
Over the past three years, she has been hospitalized more than 12 times into the city's gynecological and surgical units due to severe pain episodes.
Previous inpatient diagnoses have included
‘intestinal colic’, ‘mesadenitis’ (mesenteric lymphadenitis), ‘hematocervix (painful form)’, ‘hematometra’, ‘pelvic peritonitis’, ‘chronic appendicitis’, and ‘chronic appendicitis’.
During the hospitalizations, she received analgesics, antispasmodics, antibiotics, IV therapy, and underwent multiple cervical canal dilations and two diagnostic laparoscopies.
A prior gynecological consultation yielded the following diagnosis: ‘status post subtotal hysterectomy and electrosurgical conization of the cervix, chronic pelvic pain syndrome.’
Symptomatic pain management was recommended.
Neurological evaluation revealed vertebrogenic lumbalgia,
and recommendations included weight reduction, attending a swimming pool, analgesics for pain, and continuous administration of Lyrica (pregabalin).
During the second diagnostic laparoscopy, the surgeon noted injected blood vessels and unusual changes in the pelvic parietal peritoneum. A biopsy was taken for suspected tuberculosis. Histological examination revealed inflammatory changes in the peritoneum.
Morphological examination showed inflammatory changes: neutrophil infiltration, increased intraepithelial lymphocytes, and lymphoplasmacytic infiltration.
The patient was referred to a TB specialist, but subsequent evaluation found no evidence of extrapulmonary tuberculosis.
On physical exam, the patient exhibited typical herpetic eruption in the gluteal region.
PCR testing of a swab confirmed HSV-2 infection.
A diagnosis of ‘recurrent genital herpes, severe course’ was established, and an etiotropic suppressive herpes therapy with valacyclovir (Valvir) 500 mg once daily was prescribed for one year.
The patient reported no complaints during the 12 months of therapy.

Source text - Russian Пациентка 33 года обратилась с жалобами на привычную потерю беременности, постоянные тупые и тянущие боли в нижних отделах живота разной интенсивности.
В анамнезе 8 беременностей (1 – выкидыш, 1 – пузырный занос, 6 биохимических беременностей).
Наблюдается у гинеколога с диагнозом «хронический сальпингоофорит», по поводу которого неоднократно назначалась антибактериальная, противовоспалительная и физиотерапия.
Отмечает частые рецидивы генитального герпеса – более 6 раз в год, место излюбленной локализации – область крестца.
Во время обострения герпетической инфекции использует местное лечение в виде мази с ацикловиром.
Системная противовирусная терапия не назначалась из-за предполагаемого потенциального риска для плода ввиду ожидаемой беременности.
Во время обследования по поводу бесплодия взят аспират из полости матки.
Методом ПЦР в аспирационном материале обнаружен ВПГ 1-го типа.
В этом году была обследована у гастроэнтеролога, к которому обратилась с жалобами на примесь алой крови в кале, была выполнена фиброколоноскопия, после которой был установлен диагноз «обострение хронического язвенного колита».
Получает Салофальк, патогенетическую и симптоматическую терапию в течение длительного времени – без эффекта.
При обследовании выявлена ДНК ВПГ 1-го типа в плазме и, так как вирус обнаружен и в аспирационном материале тканей эндометрия, рекомендовано заморозить для ПЦР-диагностики часть биопсийного материала слизистой оболочки кишки.
При исследовании биопсийного материала гастроэнтеролог исключил неспецифический язвенный колит.
В биоптате слизистой кишки обнаружена как экспрессия антигенов ВПГ, так и ДНК ВПГ 1-го типа.
Выставлен диагноз «рецидивирующий генитальный герпес; герпетический колит; привычная потеря плода».
Назначена этиотропная противогерпетическая терапия в супрессивном режиме – валацикловир (Валвир) 500 мг 1 раз в день длительно.
На фоне проводимой терапии в течение 18 дней купировались клинические проявления колита и БС.
Применение валацикловира продолжено в супрессивном режиме в течение 3 месяцев с целью прегравидарной подготовки.
Translation - English A 33-year-old female patient presented with complaints of habitual pregnancy loss and persistent, varying intensity dull, tensive pain in the lower abdomen.
Her obstetric history includes eight pregnancies (one miscarriage, one hydatidiform mole, and six biochemical pregnancies).
She has been under gynecological care with a diagnosis of chronic salpingo-oophoritis, for which she has repeatedly received antibacterial, anti-inflammatory, and physiotherapy treatments.
The patient reports frequent outbreaks of genital herpes—more than six times a year, typically localized to the sacral region.
During flare-ups, she uses topical acyclovir ointment.
No systemic antiviral therapy had been initiated due to the perceived potential risks to a future pregnancy.
During her infertility workup, an endometrial aspirate was obtained.
PCR testing detected HSV type 1 in the aspirate.
Earlier this year, she consulted a gastroenterologist due to the presence of bright red blood in her stool. Fibrocolonoscopy was performed, and a diagnosis of "exacerbation of chronic ulcerative colitis" was made.
She has been on Salofalk (mesalazine), along with pathogenetic and symptomatic therapies for an extended period, without any effect.
Further testing detected HSV type 1 DNA in plasma. Given that the virus was also found in the endometrial aspirate, it was recommended to freeze a portion of the intestinal mucosal biopsy sample for PCR diagnostics.
A gastroenterologist ruled out nonspecific ulcerative colitis on biopsy examination.
The intestinal mucosal biopsy demonstrated both HSV antigen expression and HSV-1 DNA.
The following diagnosis was made: “Recurrent genital herpes, herpetic colitis, and habitual miscarriage.”
Etiotropic antiherpes therapy was initiated in a suppressive regimen with valacyclovir (Valvir) 500 mg once daily for a long term.
Within 18 days of the treatment, the clinical manifestations of colitis and pelvic pain resolved.
Suppressive therapy with valacyclovir was continued for three months as part of preconception planning.

Source text - Ukrainian Першу медичну допомогу (найпростіші медичні заходи) надають безпосередньо на місці поранення (ураження) або в найближчому укритті самі військовослужбовці в порядку само- і взаємодопомоги.
Або ж стрільці-санітари, санітари, водії-санітари, бойові медики та старші бойові медики.
Далі поранених військовиків для надання долікарської (фельдшерської) й першої лікарської допомоги направляють до медичних рот (пунктів), стабілізаційних пунктів, лікарсько-сестринських бригад цивільних лікувальних закладів.
На цьому етапі медевакуацію виконують наземним транспортом.
Наступним є військово-мобільний госпіталь (далі — ВМГ), де пораненим надають кваліфіковану медичну допомогу задля усунення тяжких, загрозливих для життя наслідків та ускладнень поранень, підготовки поранених (уражених, хворих) до подальшої евакуації і створення сприятливих умов подальшого лікування.
Медичну евакуацію поранених до ВМГ виконують наземним і повітряним транспортом.
Поранених, які потребують спеціалізованої меддопомоги (надають лікарі-спеціалісти), направляють до Військово-медичних клінічних центрів (далі ‒ ВМКЦ) регіонів, військових госпіталів, спеціалізованих закладів охорони здоров’я.
Залежно від стану здоров’я поранених, їх транспортують наземним, залізничним чи повітряним транспортом.
У разі потреби надання пораненому високоспеціалізованої меддопомоги медична евакуація виконується безпосередньо до Національного військово-медичного клінічного центру «ГВКГ» (м. Київ) усіма видами транспорту.
Кожна установа охорони здоров’я в системі Міністерства оборони України має свій профіль:
ампутація кінцівок ‒ Національний військово-медичний клінічний центр «ГВКГ» (м. Київ) та ВМКЦ професійної патології особового складу (м. Ірпінь);
нейрохірургічний напрямок — ВМКЦ Західного регіону (м. Львів);
травмування органів зору — ВМКЦ Південного регіону (м. Одеса);
термічні поранення — Національний військово-медичний клінічний центр «ГВКГ» (м. Київ).
Завдяки злагодженій роботі й чіткому дотриманню принципів медичного сортування абсолютна більшість воїнів повертається до строю.
Translation - English First aid (basic procedures) is the self-aid and buddy-aid provided immediately at the point of wounding (injury) or at the nearest place of cover.
It is also administered by combat lifesavers, corpsmen, drivers/medical aidmen, combat medics, and senior combat medics.
Wounded personnel are then transported to medical companies (aid stations) and stabilization points, and are placed under the care of medical-nursing teams in civilian health facilities to receive pre-hospital (paramedical) and emergency medical service.
At this level, MEDEVAC is conducted by ground-based transport.
The next level is a field hospital, where qualified medical care is administered, aimed at mitigating severe, life-threatening consequences and complications of injuries. It also involves preparing casualties (injured or ill) for higher care and establishing conducive conditions for further treatment. Both ground and air transport facilitate medical evacuations to field hospitals.
Casualties requiring specialized medical care (by specialist physicians) are referred to regional Military Medical Clinical Centers (MMCCs), military hospitals, and specialty health-care facilities. Depending on their health status, patients can be transported by land, rail, or air.
When highly specialized medical intervention is required, direct MEDEVAC to the National Military Hospital in Kyiv is arranged via any available mode of transport.
Each healthcare facility within Ukraine's Defense Ministry system specializes in their respective areas:
Amputations: The National Military Hospital in Kyiv and the Military Medical Clinical Center for Occupational Pathology in Irpin;
Neurosurgery: The Military Medical Clinical Center of the Western Region in Lviv;
Ocular Traumas: The Military Medical Clinical Center of the Northern Region in Odesa;
Thermal Injuries: The National Military Hospital in Kyiv.
These coordinated efforts, closely guided by triage principles, ensure the majority of service members swiftly return to active duty.

Пневмоторакс, відкритий перелом правої гомілки, осколкове поранення обох верхніх кінцівок.
Поранення отримано під час виконання обов`язків військової служби при захисті Батьківщини.
Відповідно до витягу із наказу командира військової частини [номер] від [дата] року, було проведено службове розслідування та з`ясовані об`єктивні обставини отримання позивачем поранення:
поранення солдата ОСОБА_1 відповідно до вимог ч.4 ст. 24 Закону України «Про військовий обов`язок і військову службу» сталося під час виконання ним обов`язків військової служби при захисті Батьківщини, поранення не пов`язане з вчиненням ним кримінального чи адміністративного правопорушення, та не є наслідком вчинення ним дій у стані алкогольного чи токчисного сп`яніння або навмисного спричинення собі тілесних ушкоджень.

Скарги на ниючий біль у ранах на ділянках передньої черевної стінки, лівої верхній кінцівки, лівої нижньої кінцівки.
З анамнезу відомо, що постраждалий близько 11-ти годин тому отримав поранення під час спрацювання мінно-вибухового пристрою типу "розтяжка" при виконанні завдань антитерористичної операції (АТО).
дд.мм.рррр року оглянутий лікарем-хірургом Артемівської ЦРЛ, виконане ПХО ран, видалення стороннього тіла м'яких тканин (металевого осколку) передньої черевної стінки живота, оглядова рентгенографія грудної та черевної порожнини, рентгенографія лівої верхній та лівої нижній кінцівки у двох проекціях.
На рентгенографічних знімках грудної та черевної порожнини у лівій мезогастральній ділянці виявлено стороннє тіло розміром 0,8×0,5 см, металевої щільності, орієнтовна глибина його розташування 5 см, іншої патології не виявлено.
На рентгенографічних знімках лівої верхньої та лівої нижньої кінцівок виявляються множинні сторонні тіла до від 0,3 до 0,6 см у діаметрі, металевої щільності.
Кісно-травматичних змін не виявлено.
Діагноз при надходженні:
вогнепальні осколкові множинні поєднані сліпі поранення м'яких тканин передньої черевної стінки живота, лівої верхньої кінцівки, лівої нижньої кінцівки від дд.мм.рррр року.
Стан після ПХО рани м'яких тканин передньої черевної стінки живота, видалення стороннього тіла (металевого осколку) від дд.мм.рррр року.
Поранений госпіталізований у хірургічне відділення клініки торакоабдомінальної хірургії ВМКЦ ПнР.
При огляді загальний стан задовільний.
Шкіряний покрив та видимі слизові оболонки блідно-рожеві.
Пульс 78 удари в 1 хвилину, задовільних характеристик.
Серце - тони ритмічні.
AT 125/85 мм. рт. ст.
У легенях дихання везикулярне.
Частота дихань 18 у хвилину.
Температура тіла 36,8 °C.

Translation - English Patient XXXXXX, XX y.o., was admitted to the Ukraine Ministry of Defense Northern Region Military Medical Clinical Center, after having been delivered by ambulance on dd/mm/yyyy.
Complaints of nagging pain at the wound sites of the anterior abdominal wall, left upper and left lower extremities.
According to his medical history, the casualty sustained injury approximately 11 hours ago from the detonation of a tripwire explosive device while on active duty in support of the anti-terrorist operation (ATO).
On dd/mm/yyyy, the patient was examined by an operating surgeon at the Artemivsk Central District Hospital. Wounds were initially debrided and a foreign object (metal shard) was removed from the soft tissues of the anterior wall of the abdomen. A plain thoracoabdominal X-ray was taken, along with dual-view X-rays of the left upper and left lower extremities.
The thoracoabdominal X-ray images revealed a metallic-density foreign object, measuring 0.8×0.5 cm, located approximately 5 cm deep in the left mesogastric region. No other pathology found.
Radiography of the left upper and left lower extremities revealed multiple metallic-density foreign objects, measuring between 0.3 and 0.6 cm in diameter.
No osteo-traumatic changes found.
ADMISSION DIAGNOSIS:
On dd/mm/yyyy, the patient sustained gunshot and shrapnel multiple concomitant non-perforating wounds to the soft tissues of the anterior abdominal wall, the left arm, and the left leg.
Status post initial debridement of a soft tissue wound of the anterior abdominal wall with removal of a metallic foreign body (metal shard) on [dd/mm/yyyy].
The patient was admitted into the surgical department of the Thoracoabdominal Surgery Clinic at the Northern Region Military Medical Clinical Center.
On presentation, general condition: Satisfactory.
Skin and Visible Mucous Membranes: Pale pink.
Pulse: 78 beats/min, satisfactory.
Heart: Rhythmic tones.
BP: 125/85 mm Hg.
Lungs: Vesicular breathing.
Respiration Rate: 18 BPM.
Body Temperature: 36.8 °C.
Tongue: Moist with a white coating.

Пациентка 45 лет, замужем, обратилась к гинекологу с жалобами на сухость и жжение во влагалище, тупые боли в области правой половой губы и правом бедре.
Вышеуказанные жалобы беспокоят в течение 4 лет, заболела остро, рецидивы болезни отмечаются 1–3 раза в год, за это время появилась непереносимость к холоду ввиду крайней чувствительности ног больше справа.
Акушерский анамнез: 3 беременности, 1 роды, 2 выкидыша.
Обследовалась у гинеколога, бактериальные инфекции, передаваемые половым путем, не обнаружены, парацетамол и другие нестероидные противовоспалительные препараты оказались неэффективны.
Передана для дальнейшего лечения неврологу, диагноз невролога «хроническая невропатическая боль в правой крестцовой области».
Выполнена магнитно-резонансная томография позвоночника, антитела к боррелиозу – патологии не выявлено.
Неврологом назначено лечение: амитриптилин 40 мг/сут, трамадол, 2% гель с лидокаином во влагалище – через 2 недели лечения боль усилилась и появилась везикулезная сыпь в промежности, отделяемое из везикул было направлено на исследование методом ПЦР.
Выявлен ВПГ 1-го типа.

A 45-year-old married woman presented to a gynecologist with complaints of vaginal dryness and burning, as well as dull pain in the right labium and right thigh.
These symptoms have been ongoing for four years. The condition began abruptly, and she experiences relapses 1–3 times a year. Over this period, she has developed cold intolerance due to extreme sensitivity in her feet, more pronounced on the right side.
Her obstetric history includes three pregnancies, one childbirth, and two miscarriages.
Examination by a gynecologist found no bacterial sexually transmitted infections. Paracetamol and other non-steroidal anti-inflammatory drugs were ineffective.
She was referred to a neurologist. Neurologist’s diagnosis: ‘chronic neuropathic pain in the right sacral area.’
MRI of the spine and testing for borreliosis antibodies revealed no abnormalities.
The neurologist prescribed amitriptyline 40 mg daily, tramadol, and a 2% lidocaine gel intravaginally. After two weeks of treatment, her pain worsened, and a vesicular rash appeared in the perineal area. Fluid from the vesicles was sent for PCR testing,
which detected HSV type 1.

З обох боків в голівках стегнових кісток визначаються патологічні ділянки:
визначається симптом півмісяця, площина ділянок ураження > 30% поверхні голівки із ознаками кістоподібних утворень, також ділянки підвищеної щільності, ознаки депресії до 3мм.
Суглобова щілини з обох боків звужена:
зправа до 1,4мм, зліва до 1,9 мм.
Звуження суглобових щілин крижово-клубових суглобів, субхондральний остеосклероз, "вакуум-феномен", краєві кісткові розростання на кінцях суглобових щілин.
Визначаються краєві розростання на кінцях крил клубових кісток в місці прикріплення сідничних м'язів.
Кістково-травматичних змін не виявлено.
Визначається потовщення м'яких тканин.
З боку органів тазу на рівні обстеження патологічних змін не виявлено.
КТ-висновок:
КТ ознаки асептичного некрозу голівки правої та лівої стегнових кісток III-IV ступеню.

REAGENTS.
TEST SOLUTIONS
(1) Amoebocyte lysate
Amoebocyte lysate is a lyophilised product obtained from amoebocyte lysate from the horseshoe crab (Limulus polyphemus or Tachypleus tridentatus). This reagent refers only to a product manufactured in accordance with the regulations of the competent authority.
NOTE:
Amoebocyte lysate reacts with some β-glucans in addition to endotoxins. Amoebocyte lysate preparations which do not react with glucans are available; they are prepared by removing from amoebocyte lysate the G factor, which reacts with glucans. or by inhibiting the G factor reacting system of amoebocyte lysate.
These preparations may be used for endotoxin testing in the presence of glucans.
(2) Lysate solution
Dissolve amoebocyte lysate in water for BET or in a buffer. As recommended by the lysate manufacturer. by gentle stirring.
Store the reconstituted lysate, refrigerated or frozen, as indicated by the manufacturer.
(3) Water for BET (water for bacterial endotoxins test)
Water for injections R or water produced by other procedures that shows no reaction with the lysate employed at the detection limit of the reagent. The standard endotoxin stock solution is prepared from an endotoxin reference standard that has been calibrated against the International Standard. for example endotoxin standard BRP. Endotoxin is expressed in International Units (IU). The equivalence in IU of the International Standard is stated by the World Health Organization.
NOTE:
One International Unit (IU) of endotoxin is equal to one Endotoxin Unit (E. U.).
Follow the specifications in the package leaflet and on the label for preparation and storage of the standard endotoxin stock solution.

Source text - English Tularemia, also known as rabbit fever, is an infectious disease caused by the bacterium Francisella tularensis. Symptoms may include fever, skin ulcers, and enlarged lymph nodes. Occasionally, a form that results in pneumonia or a throat infection may occur.
The bacterium is typically spread by ticks, deer flies, or contact with infected animals. It may also be spread by drinking contaminated water or breathing in contaminated dust. It does not spread directly between people.
Diagnosis is by blood tests or cultures of the infected site. Prevention is by using insect repellent, wearing long pants, rapidly removing ticks, and not disturbing dead animals.
Treatment is typically with the antibiotic streptomycin. Gentamicin, doxycycline, or ciprofloxacin may also be used.
Between the 1970s and 2015, around 200 cases are reported in the United States a year. Males are affected more often than females. It occurs frequently most in the young and the middle aged. In the United States, most cases occur in the summer.
The disease is named after Tulare County, California, where the disease was discovered in 1911. A number of other animals, such as rabbits, may also be infected.

Через 3 дня после очередного подъема температуры пациентка пожаловалась на появление затруднения при мочеиспускании и усиление боли.
В клиническом анализе мочи обнаружена гематурия, однако при этом биохимический анализ крови, посев крови, посев мочи, гинекологический осмотр, ультразвуковое исследование органов малого таза и компьютерная томография брюшной полости с контрастированием патологии не выявили.
Через 2 недели после появления затруднений при мочеиспускании пациентке показана интермиттирующая самокатетеризация мочевого пузыря для купирования острой задержки мочи.
Консультация урологом с выполнением уродинамического исследования диагностировало атонию мочевого пузыря с отсутствием ощущения наполнения.
При цистоскопии – гиперемия слизистой оболочки в области задней и боковой стенок мочевого пузыря.
По данным электромиографии, функция сфинктеров не нарушена.
В ликворе и плазме методом ПЦР обнаружена ДНК ВПГ 1-го типа.
Антитела иммуноглобулинов M и G к ВПГ 1-го типа были, соответственно, отрицательными и положительными, иммуноглобулины M и G к ВПГ 2-го типа и вирусу ветряной оспы были отрицательными.
Через 3 недели после начала обследования появились жалобы на отек левого века, офтальмологом выставлен диагноз «кератоувеит герпетической этиологии».
Назначена системная противогерпетическая терапия.
После 14 дней от начала терапии пациентка отметила отсутствие лихорадки, купирование БС, лабораторных данных за вирусемию ВПГ не было.
Через 3 недели указанные жалобы полностью купированы.

Cystoscopy showed mucosal hyperemia on the posterior and lateral walls of the bladder.
Electromyography showed normal sphincter function.
PCR testing detected HSV type 1 DNA in cerebrospinal fluid and plasma.
Serological testing showed negative IgM and positive IgG antibodies against HSV-1, and no IgM or IgG antibodies against HSV-2 or varicella-zoster virus.
Three weeks into the evaluation, the patient reported left eyelid swelling and was diagnosed by an ophthalmologist with herpetic keratouveitis.
Systemic treatment of herpes was initiated.
After 14 days of treatment, the patient reported having no fevers and having her pain symptoms under control. No lab evidence of HSV viremia.
Three weeks later, the above complaints were fully resolved.

Applicants for adjustment of status generally must complete an immigration medical examination and all required vaccinations and submit a properly completed Form I-693 signed by a civil surgeon to show they are free from health conditions that would render them inadmissible under the health-related grounds.
If you are required to submit Form I-693, or a partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485.
Otherwise, we may reject your Form I-485.
We have revised the Form I-485 instructions to make filing the two forms together a requirement.
We have made this change to reduce the number of Requests for Evidence we issue before adjudicating a Form I-485.

Результат лікування:

Висновок:
Пацієнтка дитячого віку з політравмою внаслідок бойових дій, отримала комплексне лікування з позитивною динамікою на початковій стадії.

A translator present during a high-level, closed-door meeting with Ukrainian President Volodymyr Zelensky in late 2024 was caught taking notes, something that is strictly prohibited in this type of meeting.
Interpreters are not merely translators; they are also among the few witnesses to confidential discussions between leaders. The European Commission was recently reminded of this after an incident – kept confidential until now – led to disciplinary action against a French-Ukrainian interpreter working for the institution. The incident took place on December 19, 2024, during a European Council meeting in Brussels, with Ukrainian President Volodymyr Zelensky in attendance. The findings of the internal investigation have since been forwarded to Belgian authorities, who will decide whether to pursue further investigations and whether the actions in question constitute Russian espionage.
On that December day, the European Council was hosting Zelensky. The meeting was crucial: it was intended to determine the future of European support for Kyiv, which has been under attack by Russia since February 2022.

Обставини справи
У грудні 2023 року пан Харґан звернувся до Поліційної служби Північної Ірландії (ПСПІ) із запитом, чи проходили її співробітники протягом останніх десяти років навчання в Ізраїлі або чи приймали ізраїльських представників (зокрема військових) для тренування, обміну досвідом або розвідувальною інформацією.
ПСПІ відмовилася підтвердити або спростувати наявність таких даних, посилаючись на винятки, передбачені Законом про свободу інформації 2000 року (ЗСІ):
- розділ 23(5) — інформація щодо органів безпеки (безумовний виняток);
- розділ 24(2) — національна безпека (умовний виняток);
- а також на низку інших положень.
Комісар з питань інформації підтримав позицію ПСПІ.
Апеляційна скарга пана Харґана
Аргументи апелянта:
- подібна інформація вже міститься у відкритих джерелах;
- факти співпраці з Ізраїлем мають бути предметом громадського контролю;
- посилання на конфлікт у Газі є необґрунтованим приводом для відмови у наданні інформації;
- розкриття таких даних відповідає суспільним інтересам.
Висновки трибуналу
- Виняток, передбачений розділом 23(5), поширюється на будь-яку інформацію, що належить до зазначеної категорії: орган має право відмовити у підтвердженні/запереченні, якщо це розкриває будь-які відомості про орган безпеки, зазначений у законі (наприклад, про Центр урядового зв’язку, службу контррозвідки чи зовнішню розвідку), не зважаючи на критерій шкоди суспільним інтересам.
- Навіть на перший погляд невинне підтвердження є забороненим, якщо відсутня згода відповідного органу безпеки («принцип захисту»).
- Посилання апелянта на матеріали у відкритих джерелах не є офіційним підтвердженням конкретної інформації, що запитується.
- У зв’язку із застосуванням розділу 23(5), трибунал не зобов’язаний розглядати аргументи щодо суспільного інтересу за розділом 24(2).
Рішення:
Рішення Комісара було правомірним.
- Оскарження відхилено.
