Source text - English Leucovorin can interfere with the breakdown or metabolism of certain drugs.
Following drugs are to be taken with precaution:
Folic acid antagonists:
Cotrimxazole, pyrimethamine;
Anti-epileptic substances:
Phenobarbital, primidone, phenytoin and succinimides.
5-Fluorouracil
Nucleoside analogues, e.g. Brivudin and sorivudin, which affect dihydropyrimidine dehydrogenase (DPD) activity may cause increased plasma concentrations and increased toxicity of fluoropyrimidines.
Therefore, an interval of at least 4 weeks between administration of 5-FU and brivudin, sorivudin or analogues should be kept.
In the case of accidental administration of nucleoside analogues to subjects treated with 5FU, effective measures should be taken to reduce 5-FU toxicity.
Immediate hospitalization is recommended.
Any measure to prevent systemic infections and dehydration should be commenced.
Various purines, pyrimidines, and antimetabolites have shown biochemical modulation of 5-FU in in vitro test systems.
Caution should be taken with these drugs are taken together with 5-FU.
Marked elevations of prothrombin time and internal normalized ratio (INR) have been reported in a few subjects stabilized on warfarin therapy following initiation of fluorouracil regimes.
A clinically significant interaction between the antiviral sorivudine and 5-FU prodrugs, resulting from inhibition of DPD by sorivudine or chemically related analogues.
Caution should be taken when using 5-FU in conjunction with medications which may affect DPD activity.
Increased phenytoin plasma concentrations have been reported during concomitant use of phenytoin with capecitabine or its metabolite 5-FU.
Subjects taking phenytoin concomitantly with 5-FU should be regularly monitored for increased phenytoin plasma levels.
Vaccination with a live vaccine should be avoided in subjects receiving 5-FU due to the potential for serious or fatal infections.
Contact should be avoided with people who have recently been treated with polio virus vaccine.
