Introduction
I am [Name]________________________________________________, working for the Ifakara Health Institute (IHI) which is working with ISGlobal and Unitaid to do research on how the use of ivermectin to prevent malaria might impact the environment. I am going to give you information and invite you to be part of this research. You do not have to decide today whether or not you will participate in the research. Before you decide, you can talk to anyone you feel comfortable with about the research.
There may be some words and information that is unclear to you. Please ask me to stop as we go through the information and I will take time to explain.
Purpose of the Research
You are probably aware of the BOHEMIA clinical trial that is happening in your village/hamlet. The trial is testing the use of ivermectin for the control of mosquitoes that spread malaria. When testing this new method to prevent malaria it is important to learn about how the use of ivermectin might affect the environment. When a human or animal receives ivermectin, some of it leaves the body in the stool and urine. We will collect data that will allow us to determine if ivermectin goes into and affects the environment.
Type of Research Intervention
If you agree to participate, we will collect environmental samples from the land and water where cattle graze and some of the cattle dung. We will also need to put treated cattle dungs and small nettings and marking flags on certain areas of the lands that might be used by you. It is important for this study that these nettings and marking flags will not be disturbed for the duration of the study.
Participant Selection
We have selected households to participate in this study from a list of those who are participating in the clinical trial. This selection was random. To take part the Head of Household or their substitute must have given consent.
Voluntary Participation
Your participation in this research is entirely voluntary. It is your choice whether to participate or not. Whether you choose to participate or not will not affect your participation in the ongoing BOHEMIA clinical trial or any other study that you have elected to participate in. You may change your mind later and stop participating even if you agreed earlier.
Procedures and Protocol
If you elect to participate in this study and are selected for environmental sampling, we will collect samples of soil, water, matter that settles to the bottom of the water (sediment), insects living in the bottom of the water, and dung from the land where the cattle graze. We may also ask permission to place treated cattle dung and nettings/marking flags on land for up to 10 months.
We will collect latrine samples and cattle dung samples one time before the clinical trial starts and up to four times a month in the 3 months during the clinical trial. We will collect samples of soil, water, matter that settles to the bottom of the water (sediment), and insects living in the bottom of the water, one time before the clinical trial starts and up to 8 times during the year after the clinical trial has started. All soil and water collection and testing will be on the land where cattle are grazed. We will combine the collected cattle dung samples into smaller portions with each portion placed in a cage on the land where cattle are grazed. The insects trapped inside the cages will be collected up to 9 times during a 7-month period of the study.
Sample collection:
Cattle dung samples will be taken from fresh dung piles. This will not require us to tie up an animal. We will check on the animal until it passes dung and then take a sample from the dung to check for worms.
Collection of soil, water, material that has settled at the bottom of the water (sediment), and dung samples will be conducted on cattle-grazed land after we have received your and the owner of the cattle’s permission to access and collect samples.
Duration
1 month before and 10 months after the starting of the BOHEMIA clinical trial.
Risks
We do not think there are any risks to you, your land, water or the cattle if you take part in this research.
Benefits
We expect that you and your community may have indirect benefits from taking part in this study. However, your participation will help us understand the effect using ivermectin for treating malaria on the environment.
Reimbursements
We do not expect any risks to you from participating in the study. Therefore, you will not be provided any money or goods for participating.
Costs to you
Other than your time, there is no cost to taking part in the research.
Confidentiality
We will not be sharing information about you to anyone outside of the research team. The information that we collect from this research project will be kept private. Any information about you will have a number on it instead of your name. Only a few researchers will know what your name and number is. All computers and electronic tablets are protected with a password to keep your information safe. Only authorized people will have access to this information. You will not be identified by name in any reports or publications about this study. Information about this study may be looked at by ISGlobal (the Sponsor), The Ifakara Health Institute Institutional Review Board (IHI IRB), and National Medical Research Coordination Committee (NMRCC) of National Institute for Medical Research (NIMR) and/or Unitaid (the Funder).
Right to Refuse of Withdraw
• You do not have to take part in this research if you do not wish to do so.
• You have the right not to sign this form. If you do not sign this form, you cannot take part in this research study. This is because we need your written permission to use your information
• You have the right to leave the study at any time. You will then no longer take part in this study and we will not use your information. If you would like to leave the study, please tell a member of the study staff. You do not need to explain why you want to leave the study.

Ukusanyaji wa sampuli
