The standard of care for acute lymphoblastic leukemia is chemotherapy. The authors propose a project to determine whether immunotherapy can be employed in treating ALL. The investigators propose to identify the tumor specific CD8 positive lymphocytes in peripheral blood, followed by phenotypically characterizing them for expression of proteins such as PD-1, LAG-3, and TIM-3. The authors also plan to define the profile of cytokines released by the CD8 cells in vitro after stimulation. Finally, the authors propose to analyze whether chemotherapy influences patient specific clones.
This proposal is well written and the objectives are clearly stated and attainable. These results may have an important impact in managing patients with acute lymphoblastic leukemia, either as a first-line therapy or in refractory cases.

El tratamiento estándar para tratar la Leucemia Linfoblástica Aguda (LLA) es la quimioterapia. Los autores proponen un proyecto para determinar si la inmunoterapia puede ser usada en el tratamiento de LLA. Ellos plantean identificar los linfocitos positivos CD8 específicos del tumor en sangre periférica, seguido por caracterización fenotípica de los mismos a través de expresiones de proteínas como PD-1, LAG-3 y TIM-3. Los autores también plantean definir el perfil de las citoquinonas liberadas por células CD8 en vitro después del proceso de estimulación. Finalmente, se plantea analizar si la quimioterapia influencia clones específicos del paciente.
Esta propuesta está bien estructurada. Los objetivos están claramente definidos y son alcanzables. Estos resultados podrían tener un importante impacto en el manejo de pacientes con Leucemia Linfoblástica Aguda ya sea como terapia de primera línea o en casos refractarios al tratamiento.
