Source text - English Mr. xx is a xx year old male patient, who has had a gastrointestinal disorder of long duration.He was seen by me in 1996. Prior to that, he had a history of multiple gastric ulcerations of a few years' duration. Subsequent to my evaluation, he was treated with medication.He did have some persistent ulceration, and subsquently they were able to heal it. In 1998, he had abdominal pain, and colonoscopy did reveal some nonspecific patchy colitis. He was found to have ulceration in 1999. Subsquent to therapy, it slowly healed. He has been having recurrent episodes of GI bleeding. He did undergo capsule endoscopy of the small bowel, which revealed fragile capillary syndrome causing anemia. He was hospitalized in 2000 for bleeding ulcer from the stomach. It was conservatively managed. In 2004 he again required a hospitalization for a bleeding gastric lesion and subsquently recoverd. In 2006 patient was disabled. In 2007, he was hospitalized for recurrent bleeding, subsquently he was noted to have small bowel bleed, which required a local resection. It was found to be from a vascular hernia. subsquent to that, he developed a ventral hernia. Currently, he does have a ventral hernia giving him a weak feeling and abdominal discomfort.

Translation - Portuguese XXX, de XX anos de idade, do sexo masculino, paciente que apresentava uma perturbação gastrointestinal de longa duração. Este paciente foi consultado por mim em 1996, mas antes de ter sido visto por mim, j a lhe tinham sido diagnosticadas múltiplas ulcerações gástricas, com poucos anos de duração.
Subsequentemente á minha avaliação, ele foi tratado com a respectiva medicação. O paciente teve alguma ulceração persistente que, subsequentemente tinha sido tratada por outro colega.
Em 1998 ele sentiu fortes doeres abdominais, em sequência disso, foi-lhe feita uma colonoscopia, que revelou algumas não especifícas manchas irregulares e colite. Em 1999 foram também verificadas ulcerações. Após, terapia houve cicatrização lenta. O paciente tem tido alguns episódios de sangramento. Foi também submetido a uma endoscopia do intestino delgado, que revelou fragilidade capilar, sendo este o síndroma que causa a anemia.
Foi hospitalizado em 2000 devido a sangramento de uma ulcera no estômago. este episódio foi bem tratado.
Em 2004 voltou a ser hospitalizado devido a uma lesão, e sangramento gástrico, à qual recuperou bem.
Em 2006 o paciente ficou incapacitado. Em 2007 voltou a ser hospitalizado devido a sangramento recorrente, e subsequentemente também lhe foi detectado sangramento nas fezes ao qual foi necessária uma pronta intervenção, e com uma recepção local. Verificou-se que este sangramentonas fezes era devido a uma hérnia vascular. Subsquentemente a tudi isto, ele desenvolveu uma hérnia abdominal. Presentemente ele tem uma hérnia abdominal que lhe dá uma sencação de fraqueza e desconforto abdominal.
