Judicialization of the right to health
The text is about patients who take claims for non-approved therapies to court to force the health system to pay for them. It's a growing practice in Latin America, used most often for cancer chemotherapy. In this case, the country is Colombia.

The idea of "full-time" is essential if you are thinking about the individuals who drive the judges around.
Permanent chauffer service
Permanent chauffer-driven cars
These are other possibilities, especially if the idea is always being able to have someone drive you around, but not necessisarily the same driver each time.

Note from asker:
Thanks, Cinnamon. In this case, it's actually the patients who are appealing to the court to get payment approved for the benefits. But even so, your point is well taken and I think it conveys the idea nicely. |

permanent chauffeurs
Or "drivers", if you prefer, but perhaps "chauffeurs" makes it clearly. That is, people to drive you around all the time, on the grounds that you can't drive yourself. Given the context, this is what it suggests to me.

Sorry: "makes it clearer". My typing is getting increasingly inaccurate; it's rather alarming.

personal drivers
I think "personal" better than "permanent" and I don't think the peaked cap image of "chauffeur" is needed here. Otherwise I agree with Charles' reading of this given the context.

in terms of the "permanent" point, well, I really think it sounds clunky in English. It is increasingluy common for (infirm/disabled) people to have personal assistants and/or drivers on a part-time or full-time basis and the positions are usually contracted so perhaps "full-time" would get the idea across, provided you know it's full-time position of course!

: I think this is a good idea, though I'm not sure about sacrificing "permanent"; I feel the ongoing nature of it is an important element. "Chauffeurs" don't have to wear peaked caps!

thanks Charles:-) No, you're right, they don't, but I'm afraid that's the image conjured up in my mind, and I'm sure for others, of the hoi polloi/celebrities/limos/Rolls/weddings!

: In think "permanentes" has a different meaning, rather than "personal".

Not saying "parmanentes"=personal but that's what the role/position is of this individual imo.
