English to Italian: Adrenal suppression due to an interaction between ritonavir and injected triamcinolone: a case report General field: Medical

Two HIV-1 infected patients developed signs and symptoms consistent with adrenal suppression after being exposed to intra-articular triamcinolone acetate while also receiving ritonavir as part of their highly active antiretroviral therapy. Laboratory evaluation confirmed secondary adrenal suppression in both cases. Both patients recovered without the need for chronic replacement steroids. Adrenal suppression has been described as an adverse outcome in patients treated with fluticasone an concomitant ritonavir. In the reported cases, the adrenal suppression likely developed as a result of increased systemic concentrations of triamcinolone due to an inhibition of cytochrome p450 3A4 metabolism. Practitioners of HIV medicine should be aware of the potential negative interaction of injected triamcinolone and ritonavir.

Due pazienti con infezione da HIV-1 hanno sviluppato segni e sintomi coerenti con un quadro di soppressione surrenalica, in conseguenza di una terapia antiretrovirale altamente attiva, a base di un’associazione di triamcinolone acetato intra-articolare e di ritonavir. Indagini di laboratorio hanno confermato una soppressione surrenalica secondaria in entrambi i casi. I pazienti sono guariti senza bisogno di assumere cronicamente ormoni steroidei sostitutivi. La soppressione surrenalica è stata descritta come esito avverso in pazienti che ricevevano congiuntamente fluticasone e ritonavir. In tali casi documentati è probabile che la soppressione surrenalica sia stata indotta dall’aumento delle concentrazioni plasmatiche di triamcinolone, prodotte dall’inibizione del metabolismo del citocromo p450 3A4. I medici che trattano pazienti con infezione da HIV devono tener presente la potenziale interazione negativa tra triamcinolone iniettato e ritonavir.

Data sources
We focused on prospective studies investigating the
association between adherence to a Mediterranean diet
and health outcomes. We searched PubMed, Embase,
Web of Science, and the Cochrane Central Register of
Controlled Trials databases up to 30 June 2008, using a
search strategy that included both truncated free text
and exploded MeSH terms. MeSH headings included
“Mediterranean”, “diet”, “dietary pattern”, “disease”,
“health”, “cardiovascular disease”, “cerebrovascular
disease”, “coronary heart disease”, “degenerative
diseases”, “cancer”, “neoplasm”, “prospective”, “follow-
up”, or “cohort”, and their variants. The search
strategy had no language restrictions. We also consulted
references from the extracted articles and
reviews to complete the data bank. When multiple
articles for a single study were present, we used the
latest publication and supplemented it, if necessary,
with data from the most complete or updated publication.
We assessed the relevance of studies by using a
hierarchical approach based on title, abstract, and the full
manuscript.

La strategia di ricerca non aveva restrizioni rispetto alla lingua. Sono stati, inoltre, consultati i riferimenti bibliografici degli articoli estratti e le recensioni per completare la banca dati.
